Job Summary:This position will report to the Program Director and is an integral part of the SSG Fox Team with include both the data and case management components, which works collaboratively to ensure all contractual documentation standards and reporting requirements are met. This role understands the importance of data collection and entry to the functioning and improvement of programs. This role identifies trends and manipulates data to demonstrate program outcomes. This position is critical in supporting Programs data as a part of daily activities at the agency. This position responsible for creating partnerships and linkages, as well as identifying individuals in need of services for themselves or loved ones. The Data Community Outreach Case Manager will work a conventional work week that includes evening hours and may require weekend work. In identifying individuals in need of community-based suicide prevention efforts; the Community Outreach Case Manager will work collaboratively with other organization to improve referral pipelines for referrals and care within VOACC's Suicide Prevention services. JOB DESCRIPTION •Provide or arrange for support and wraparound services including connections to all eligible VA and mainstream resources to support. •Communicate effectively (under confidentiality rules) with service team to rapidly address and correct issues with services or supports - especially living skills, employment, education, family and socialization. •Participate in program and professional supervision, attend required staff and Project Management Team meetings and trainings, and contribute to team-based collaborative planning. •Attend monthly community outreach meetings. •Assist neighbors experiencing homelessness with accessing resources and making referrals; continue to engage with unsheltered homeless neighbors until shelter or permanent housing is obtained. •Maintain computerized client records, daily activity logs, mileage logs, and other reports as directed. •May drive a van/shuttle; complete daily vehicle inspection report on van/shuttle; may clean van/shuttle; and may maintain all safety conditions in the van/shuttle. •Develop a relationship with neighbors through frequent outreach visits to foster trust building and supports participation in programs. •Research community resources that benefit veterans. •Provide advocacy for veterans when they encounter barriers. •Foster integration of program throughout existing departmental and regional programs and services, including homeless services, veteran's services, behavioral health, etc. •Travel throughout community and local sites. •Other duties as assigned.PRINCIPAL ACTIVITIES:
  • Manage all aspects of data entry and enter, export, and import service delivery, intake, and outreach data into VA DASHBOARD, HMIS, AWARDS, KPIs and other systems as required
  • Ensure monthly reports and HMIS repository upload as required by the program are completed
  • Conducts routine data review and verification and ensures corrections/clarifications are made in a timely manner (Audits)
  • Work directly with team members to regularly perform data entry, quality assurance

  • Audit cases and documentation (self-audit and staff audit)
  • Proofreading skills to see how data and facts can be linked together to create more meaningful conclusions[TM1]
  • Review data files and provide guidance and direction to management on how to improve or maintain data quality.
  • Conduct weekly Data team meetings (internal and/or external)
  • Assists with the development of planning, evaluation and implementation of policies and procedures in support of the Data Plan with the program
  • Maintains data records and prepares forms, reports and correspondence related to program
  • Attends and participates in all appropriate meetings and presentations to explain the program data
  • Facilitates and encourages the exchange of program data information with local, county, and regional agencies
  • Maintains and updates knowledge through conferences, seminars and in-service trainings as determined in consultation with department leadership
  • Plans and conducts training as assigned
  • Provide Monthly summaries and data reports as required by supervision
  • Participate in program and professional supervision, attend required staff meetings and Management Team meetings and trainings, and contribute to team-based collaborative planning
  • Coordinate outreach efforts for the Veterans Program including but not limited to: establish an outreach plan to target the three categories of eligible participants as outlined in the Staff Sergeant Parker Gordon Fox Suicide Prevention Grant Program's VA program guide, develop a set schedule to visit the VA, shelters, and other places that veteran families are likely to congregate, and develop communication/ updates with the VA and other community providers.
  • Screen potential program participants for program eligibility using VA-approved forms and follow up with applicable agencies to verify information including veteran and income status.
  • Provide the following SSG Fox SPGP services as outlined in the program guidelines:
  • Outreach to identify those at risk of suicide
  • Baseline Mental Health Screening (Participants over 18 years of age)
  • Education on suicide risk and prevention to families and communities
  • Provision of clinical services for emergency treatment
  • Case management services
  • Peer support services
  • VA benefits assistance for eligible individuals and their families
  • Assistance with obtaining and coordinating other benefits provided by the federal government, a state or local government, or an eligible entity
  • Assistance with emergent needs relating to health care services, daily living services, personal financial planning and counseling, transportation services, temporary income support services, fiduciary and representative payee services, legal services to assist the eligible individual with issues that may contribute to the risk of suicide, and childcare
  • Non-traditional and innovative approaches and treatment practices, as determined appropriate by VA
  • Other services are necessary for improving mental health status and well-being and reducing the suicide risk of eligible individuals and their families as determined appropriate by VA.
QualificationsREQUIREMENTS•Bachelor's degree in Human Services or related field required. •3 to 5 years of experience working in the field of outreach with experience working with Veterans or people who have mental health, substance use, or IDD concerns.
  • A minimum of two years of experience working with data systems, data entry & data analysis in homeless programs or services preferred.
•Demonstrated ability to explain an agency's purpose, programs, and operations. •Knowledge of available community resources. •Self-starter who is mission driven and committed to helping people connect with our services. •Excellent Computer Skills in Microsoft Office and office products. [TM1] •Excellent written and verbal communication skills, networking and organizational skills. •Proven ability to for conflict resolutions •Bilingual in any language a plus
  • Have relevant experience in data input/extract.
  • Process, enter, and manage large amounts of data across a variety of software systems, produce reports, and support all quality assurance/quality improvement efforts
  • Proficiency with Microsoft Office 365 including Word, Excel, PowerPoint, Outlook and SharePoint preferred
  • High level of administrative self-sufficiency including computer proficiency
  • Ability to multi-task and prioritize many projects with a high level of customer service and confidentiality
  • Ability to travel when necessary to engage partners and solicit prospects
  • Ability to work independently and collaboratively required
Training
  • This position goes through a training period spanning at least 90 days. During this time, they will work closely with the Program Director and assigned Team Members to become familiar with daily operations and expectations.

EFFECTS ON END RESULTS:
  • Maintain strong data quality with a strong understanding of quality metrics, analytics, and reporting while communicating with senior management, HMIS providers and The Program
  • To provide community stabilization support
  • Provide immediate mental health services to client in mental health crisis states
  • Decrease hospitalization utilization rates in the community
  • Decrease law enforcement contact with mental health clients and reduce recidivism rates to the prison systems
