DUTIES AND RESPONSIBILITIES:
* Writes and updates Staff Action Plans (including Valued Outcomes) and Individualized Plans of Protection.
* Reviews and monitors the implementation of Staff Action Plans, IPOPs, and other services (as well as documentation of same) for accuracy, progress, and quality assurance purposes.
* Writes Monthly Summary of Individuals' progress based on review of daily notes, progress notes, discussion with staff and direct observation/contact.
* Completes a pre-meeting with Day Habilitation team prior to all Life Plan meetings or semi-annual reviews.
* Attends Life Plan meetings and other meetings as appropriate/assigned for the individuals assigned.
* Joins with Day Habilitation Specialist and Direct Support Professionals in developing daily activities for room(s) assigned.
* Ensures that all Life Plans are received, forwarded, and filed per agency policies and procedures.
* Assist with monitoring and entering attendance information as needed.
* Manages and maintains individuals' monies including all necessary receipts and ledgers for program outings in an accurate manner and per program procedures.
* Provides direct service to individuals as needed and directed, including participating in community activities.
* Provides supervision and training to Direct Support Professionals as assigned.
* Will complete all mandatory trainings and other trainings assigned.
* Keeps all agency business and communications within the agency and communicates through the proper channels of the chain of command and appeals process.
* Responsible for inventory of assigned materials and completing purchase orders for needed items.
* Responsible for the general housekeeping of assigned areas.
* Conducts self as a member of a therapeutic team in regards and relation with all Program Individuals of the agency and maintains this professional relationship at all times. No unapproved activities with Program Individuals are permitted.
* Conducts self in such a manner as to meet agency policies and standards at all times.
* Understand and applies learning that all Program Individual's information is confidential and not to be discussed or shared outside of the agency, to include public, social settings.
* Participates in investigations in any matters related to the Agency, Staff, or program individual.
* Maintains an objective approach to job duties, free of personal values or philosophies.
* Understand and applies learning that proselytizing on the job or using Agency information regarding the program individuals to promote one's own personal philosophy is prohibited both during and after working hours.
* Adheres to the Code of Conduct for Staff Members and Volunteers which is included in the Staff Handbook.
* Performs any other duties as deemed necessary by Coordinator of Day Habilitation Services, Director of Day Habilitation Services, Associate Executive Director of Day Services, and Executive Director.
* Punctuality and attendance during scheduled work hours are essential functions of this position.
* This position requires a valid NYS Drivers License with an acceptable driving record as determined by Arc of Genesee Orleans. While driving an Agency vehicle or driving a personal vehicle on Agency business, all NYS laws and Agency policies pertaining to driving must be adhered to at all times.
EDUCATION/EXPERIENCE REQUIREMENT:
BA/BS in related field and six months experience working with Individuals with Disabilities
OR
Associates Degree in related field with three (3) years experience working with Individuals with Disabilities.
Lifting requirement: Must be able to lift minimum of 30 pounds.
Valid NYS Drivers License with an acceptable driving record as determined by Arc of Genesee Orleans.
