PathForward, IncDay Program Monitor (HSC)Job DescriptionPosition Summary:The Day Program Monitor is responsible for monitoring the activities of residents of the Day/Outreach/Hypothermia/shelter/Center Programs and toensure compliance with program rules and expectations. This position is also responsible for ensuring the safety of residents and the security and sanitation of the program. The Day Program Monitor reports to the Sr. Director of HSC Programs.The Day Program Monitor (HSC)will provide direct program support to residents and work collaboratively with Case Managers to help ensure successful completion of Individual Housing Plan (IHP).Key Responsibilities• Actively seek out and work with homeless persons throughout Arlington County initiating the process of engagement to support additional services. Build trust and rapport by offering emergency assistance such as food, clothing, sleeping bags, and shower vouchers.• First point of contact for many clients and community members, must maintain a friendly, courteous attitude and have good communications skills, both in person and by telephone.• Participates in Outreach programs street homeless calls.• Participates in planning of community room policies and direct operation of the community room to ensure effective and efficient client service delivery.• Directing operation of the Day/Outreach Program to ensure effective and efficient client services delivery during walk-in hours, assist clients in accessing available services.o Informs individuals of all available services offered at the Day/Outreach Program.o Assisting persons in crisis to ensure safety.o Receives and record donations - (provide donation slips to donors for tax purposes)o Maintains a daily log of client contacts and services.o Provides an outreach response to calls from members of the community including religious institutions, citizens and businesses regarding sightings of homeless individuals to the Manager of Day Program and Outreach.o Ensures safety of clients and staff in fire and other emergencies.o Ensures clients compliance with agency policy and rules.o Assists clients within limits in processing issues and problem-solving.o Checks for contraband in facility through general observation, room or package/purseo Conducts periodic inspections of the facility and grounds through periodic rounds and CCTV monitoring to ensure security of facility.o Enforces periodic fire drills and perform other safety duties as directed to ensure safety of clients, including implementing emergency evacuation protocol.o Answers telephone and route calls or messages to appropriate staff. Respond to in-person and telephone inquiries from the public.Position: Day Program Monitor (HSC) FLSA Status: Non-ExemptManager: Sr. Director of HSC Programs Location:Job Type: Full Time Department: HSC Sheltero Receives, accounts for and secures all donations received during the shift, as well as provide donation receipts to donors upon request.o Provides daily facility oversight for such items as ordering of supplies (in liaison with the Manager of Day Program and Outreach).o Receives deliveries and incoming mail and distribute accordingly.o Reports needed repairs and pertinent items in relation to the facility that may affect services at Day Program to Facilities Maintenance at ext. 4422 and theSr. Manager of Human Resources and Administration.o Maintains cleanliness of Dinning Area, Lobby, Front desk, and Client Restrooms.• Performs informal intake assessments to determine individuals' social and economic needs, noting observations on possible mental health and substance abuse issues through the administration of the VI-SPDAT as well as other engagement tools.• Coordinates with other shelter and day program staff to present moving on classes and other activities offered to Day/Outreach Program clients.• Develops and implement Day Programming for clients 20-25 hours per month with the assistance of The Volunteer & Special Events Coordinator.• Completes an intake for each client who enters Day/Outreach Program and street homeless in accordance with the CAS System in HMIS.• Coordinates all efforts towards housing (Day/Outreach clients) in collaboration with DHS and community partners.• Coordinates all services and resources available to homeless individuals such as:o accessing available serviceso Identity documentso benefitso health services• Works as part of a team, meeting regularly to share information, strategies, mutual support, and ideas with a primary goal to address the long-term needs of homeless individuals including access to permanent housing.• The Day/Outreach Monitor will provide direct program support to clients and work collaboratively with Case Managers to help ensure that clients acquire skills to end homelessness. Maintain contact to provide encouragement, information and referrals relevant to the person's needs including bi-lingual material when needed.• As clients are engaged, refer to Day/Outreach Case Manager for assistance in accessing needed services including treatment for medical, mental health and substance abuse issues as well as social service, housing/shelter and employment services.• Maintains client confidentiality at all times.• Adhere to NASW Code of Ethics.• Other duties as assigned.Supervisory Responsibilities:• None.Qualifications and Skills• Ability to work with persons with serious mental illness and/or substance addiction.• Ability to prepare written reports.• Ability to monitor and direct program activities efficiently.• Ability to consistently ensure the safety and security of program operations.• Proficient knowledge of Microsoft Word, Excel, and similar software.• Proficient knowledge of Homeless Management Information System databases.• Ability to communicate in both Spanish and English preferred.• Demonstrated ability to exercise good judgement.• Consistently treat all clients and staff with respect.• Excellent written and verbal communication skills.• Excellent interpersonal and organizational skills.• Reliable and dependable and able to handle multiple tasks and deadlines,• Valid driver's license.Experience and Education Requirements• Bachelor's degree in Human Services or related field preferred.• Minimum 2 years' experience in community based social services or other related experience working with homeless adults, mentally ill, and/or substance abusers.• CPR/First Aid Certification must be acquired and maintained once employed.Physical Requirements:• Work is performed in a shelter.• Must be able to lift a minimum of 30 pounds.• Work is subject to frequent interruption.• Subject to work flexible hours that may include weekends, overnights, evenings, and holidays.• Subject to work in a variety of weather conditions.8:00am-4:00pm8 hours
