**DISCLAIMER: THIS POSITION WILL BE WITH A PRIVATE AGENCY, CONTRACTED WITH CARTERET COUNTY PUBLIC SCHOOLS TO WORK AT BRIDGES SCHOOL. THIS POSITION WILL NOT BE EMPLOYED BY CARTERET COUNTY PUBLIC SCHOOLS. Minimum Qualifications: Getting Ready, Inc. Day Treatment Qualified Professional Job Description Meets one of the following for a Qualified Professional according to staff definitions 10A NCAC 27G:
  • an individual who holds a license, provisional license, certificate, registration or permit issued by the governing board regulating a human service profession, except a registered nurse who is licensed to practice in the State of North Carolina by the North Carolina Board of Nursing who also has four years of full-time accumulated experience with the population served; or
  • a graduate of a college or university with a Masters degree in a human service field and has one year of full-time, post-graduate degree accumulated with the population served; or
  • a graduate of a college or university with a bachelor's degree in a human service field and has two years of full-time, post-bachelor's degree accumulated experience with the population served; or
  • a graduate of a college or university with a bachelor's degree in a field other than human services and has four years of full-time, post-bachelor's degree accumulated experience with the population served
AND is required to have the skill, knowledge, and experience with child/adolescent MH Within 30 days of employment to provide this service, or timeframes required by NCDHHS, each staff member must complete a minimum of 20 hours of training specific to components of the Day Treatment service definition, the clinical model(s) or evidence-based treatment(s) consistent with best practice implemented, crisis response, Person Centered Thinking, Systems of Care (SOC) Child and Family Team training. *Personnel File will contain documentation of all required components as indicated in P&P. QP will follow the training for new employees as indicated through the training plan implemented by the Training Director. Specific Duties and Responsibilities:
  • Coordinate initial and ongoing assessments and discharge transition
  • Develop the Person-Centered Plan,
  • Ongoing monitoring of the implementation of the Person-Centered
  • Revise and update the PCP as
  • Provide additional case management functions of linking, arranging for services and
  • Participate and/or coordinate in treatment planning
  • Keep in close contact with families and people involved in the clients care to encourage and promote family
  • Providing first responder crisis (proactive/reactive) plans, which may require face-to-face and/or telephonically services at all times (24/7/365) within 2
  • Assure proper administration of all medication and proper documentation. (if applicable)
  • Develop the monthly activities schedule with the
  • Documenting the child's progress and the effectiveness of the strategies and interventions outlined in the PCP.
  • Collaborating with other medical and treatment
1) Within 30 days of hire:
  • 3 hours Day Treatment service definition required components
  • 3 hours crisis response
  • 3 hours PCP Instructional Elements (for QPS responsible for PCPs)
2) Within 90 days of hire (by 6/30/2011 for existing providers):
  • 12 hours Person Centered Thinking
  • 11 hours Introduction to SOC
  • Minimum 24 hours specific to modality selected for service delivery
  • Annually - Follow up or ongoing training required by modality selected for service delivery (minimum of 10 hours)
***This position is classified as exempt under the FSLA 29 CFR part 541 and paid at a rate inclusive of all billable and non-billable duties.***
