Job Description:

*Note* MUST have a MSW or Client (no exceptions), as well as must be LCSW or LMSW prior to start date. We need experience Social Workers, at least 2 yrs experience in acute care.

The successful candidate may be eligible for a *** sign onbonus to be paid within first year of a two-year commitment.

THECARE MANAGEMENT MODEL:

Our Care Management Triad TeamModel is a collaboration between the following:

• RN Case Manager - manage patient care and drive patient progression and establish a discharge plan.
• Social Worker resolve psycho-social barriers and supports discharge needs.
• Utilization Management review patient status for appropriateness and anticipated payer coverage.

EDUCATION AND EXPERIENCE REQUIREMENTS:

• Masters degree in Social Work (MSW) or Master of Social Services degree (Client) from an accredited graduate school required prior to hire date.
• Licensed CSW or MSW as required by the state of Delaware prior to hire date.
• Acute Care Social Work experience preferred.
• Maternal-Child position requires experience working with neonates, children and families

SPECIAL REQUIREMENTS:

CCM preferred within 18 months ofeligibility.
