Position: Diabetes Program Registered DietitianPosition Number: 479 LDivision:HealthDepartment:Population Health Reports To:Diabetes Program AdministratorLocation: LincolnExempt/Non-Exempt:Non-exemptSalary:24.61-37.52Classification:Fulltime (Grant funded)Summary:Incumbent will provide medical nutrition therapy and nutrition education within the Registered Dietitian scope of practice and as directed by the Diabetes Program Administrator. This will include providing diabetes education classes.Essential Responsibilities and Duties:
Provides nutrition screening, nutrition education, and medical nutrition therapy in disease prevention and management to clients on an individual and group basis while adhering to the Standards of Practice in Nutrition Care for Registered Dietitians.Assists in developing the Diabetes Program annual proposal, plan of care and annual reports.Participates in the Quality Improvement within the health department in order to improve continuity of care of patients.Participates in data entry/analysis of diabetic patient records including, diabetes risk indicators, diabetes incidence and care.Collaborates with medical team and pharmacy to provide remote patient monitoring services for chronic disease management of patients. Participates in various trainings or meetings that involve obtaining support for the Diabetes Program, keeping updated on current nutrition, dietetics, and diabetes knowledge.Participates in activities promoting healthy eating and physical activity as means of preventing diabetes and other health related problems associated with diabetes. These activities will be offered during health fairs, youth camps, powwows etc.Implements the instructions as directed by the Diabetes Program Administrator and the policies of the Ponca Tribal Council, which is the governing body of the Ponca Tribe, and complies with policies, procedures, orders, rules and regulations of the Tribe. Complies with all ethical and professional codes applicable to the profession of dietetics and all federal, state, municipal and tribal laws and ordinances related or regulating the actions of the Dietitian.Maintains and preserves records and submits on a monthly basis the number of client visits, client education/nutrition visits and diabetes prevention activities to the Diabetes Program Administrator and Chief Executive Officer.Maintains an unrestricted license by the Commission on Dietetic Registration by the State of Nebraska.Recruits Native Americans with diabetes and/or at high risk of diabetes to participate in the diabetic control and prevention programs offered by the PTN.Provides transportation to Native Americans to attend the Diabetes Program and other health related programs.Must be willing to obtain continuing education on all aspects of diabetes prevention management in order to be compliant with required CEUs.Must be willing to learn and follow the Indian Health Services Standards of Care for Adults with Type 2 Diabetes.All appropriate data and records will be kept confidential, accurate, current and complete.Other duties as assigned.Relationships and Authority:Incumbent will have no supervisory authority.Working Environment and Physical Demands:This position is located within an outpatient medical clinical setting. Works throughout the clinic area. Sits, stands, bends, lifts and moves intermittently during work hours. Employee is subject to emotionally upset patients, family members, personnel and visitors. Qualifications:Minimum:Registered Dietitian and Licensed Medical Nutrition Therapist in the State of Nebraska.Able to obtain First Aid and CPR certification within three months of hire date, if not already certified.Computer proficiency with Microsoft Word, Excel, PowerPoint and Access programs.Excellent written and oral communication skills.Valid driver's license and be insurable.Successfully pass drug and alcohol testing, motor vehicle, education, character, and criminal background checks.Able and willing to travel.Preferred:Certified Diabetes Educator or qualified to become certified within 12 months after hire date.One or more years of experience working with Native Americans or Native American organizations.Knowledge and understanding of Public Law 93-638, Public Law 101-484 and the Privacy Act of 1974.Preference will be given to Ponca Tribe of Nebraska members or members of a federally recognized tribe.
