Provides individualized diabetes education based on identified patient needs.
  • Creates comprehensive nutrition care plans for patients by identifying pertinent data from medical/diet/social histories, reviewing medications and medical interventions, and through patient interaction to ensure appropriate patient assessment.
  • Integrates pathophysiology, calorie, nutrient and fluid intake vs. patient needs and the clinical outcome goals into nutrition recommendations and interventions to ensure positive patient outcomes.
  • Provide teaching utilizing teach back method to ensure patient understanding.
  • Provides nutrition education to patients and families through counseling, education, and training materials to ensure compliance with the established nutrition care plan.
  • Provide instruction, demonstration, and teach back on glucose monitors.
2. Assists patients to identify and set personal goals for improvements.
  • Assists patients to identify and set personal nutrition and exercise goals.
  • Provide frequent follow up support to patients to ensure goal success. Frequency of follow up to be determined by individual patient needs.
3. Identifies and addresses barriers that prevent patient success.
  • Responsible for identifying barriers to obtaining patient care goals such as financial barriers, physical barriers, medication adherence, knowledge deficit, etc.
  • Responsible for addressing barriers to patient care goals and assisting patients with resolution to the barriers.
  • Responsible for follow up to ensure successful resolution to barriers that prevent patient success.
4. Charts in the electronic medical record.
  • Accurately charts diabetes education in the Electronic Medical Records (EMR) utilizing the AdHoc Diabetes Outpatient Individual Form.
  • Provide timely and accurate patient updates via Messaging through the EMR to physicians and the health coach. This includes bi-monthly blood sugar updates, etc.
5. Follows up with High Risk patients.
  • Responsible for contacting all identified high risk patients in a timely manner. The list of high risk patients is provided monthly by the health coach.
  • Responsible for providing frequent and continuous support to the high risk patients. High risk patients should be provided support and follow-up on a weekly basis.
  • Ensure high risk patients are scheduled for physician follow-up appointments per pre-established guidelines.
  • Responsible for keeping the physician and/or health coach notified of any pertinent patient issues such as failure to contact patient, changes in patient progress, etc.
  • Responsible for follow-up on high risk patients who do not present for appointments with the diabetic educator or physician.
6. Assists with educating other Clinic Diabetic Educators, Health Coaches, and other clinical staff to facilitate continuo learning and development.
  • Willingness to adequately prepare and present current educational information for other staff.
  • Assists with ongoing nutrition education material updates by researching, reviewing, and utilizing additional resources appropriately to ensure educational material is current.
7. Communication Collaborates with physicians and health coach to provide quality care of the patient with diabetes.
  • Collaborates with health coach to assess plan, implement and evaluate patient education.
  • Participates in interdisciplinary activities to ensure adequate communication is provided to develop and update the patient care plan.
8. Provides appropriate care specific to the age of the patient to ensure understanding and comfort level of treatment as outlined in the Age Specific Criteria .Schedule:Monday through Friday 8 am to 5 pmJob Description:Job RequirementsEducation
  • Bachelors Degree required, preferably in Nutrition and Dietetics.
Experience
  • 3 years of previous clinical experience preferred.
License/Certifications
  • Certified Diabetes Care and Education Specialist (CDCES) through the Association of Diabetes Care & Education Specialists (ADCES) preferred.
  • Registered Dietician (RD), Registered Dietician Nutritionist (RDN) or Registered Nurse (RN) required.
Skills/Knowledge/Abilities
  • Knowledge of patient education.
  • Skilled in critical thinking for determining alternative solutions or solving problems required.
  • Strong verbal communication skills for positive interaction with patients, physicians and other health care team member.
  • Skilled in operating a computer, including a computer mouse and keyboard, required.

  • Able to keep patient information confidential required.
