Every employee takes a person-centered approach that exemplifies the ICARE values (Integrity, Compassion, Accountability, Respect, and Excellence) through empathic communication and partnerships between all persons.Job DescriptionPURPOSE OF POSITIONThe Registered Dietitian will provide nutrition care through assessment, nutrition diagnosis, intervention, monitoring and evaluation of patients admitted to the New York City Health + Hospitals Certified Home Health Agency ('Agency'). S(H)e will be responsible for assuring that nutrition care is provided and documented in accordance withthe Agency's policies and procedures as well as regulatory guidelines.Duties & ResponsibilitiesAREAS OF RESPONSIBILITIES:1. Conducts assessments of patients' nutritional needs. Interviews patients and/or their families/caregivers to obtain history of food habits, preferences and treatment goals. Interprets laboratory, physical, and social data in medical records to inform nutritional assessments.2. Prepares nutritional assessments, nutritional care and discharge plans, and documents nutritional data and follow-up plan of care in the medical record. Ensures that nutritional assessment and care plan are specific to patients' diagnosi(e)s and in accordance with provider orders.3. Ensures that the plan of care addresses all problems identified during the evaluation or demonstrates rationale for not doing so. Regularly reviews dietary needs with patients. Counsels patients and/or their families in the preparation of special therapeutic or modified diets at home.4. Monitors patients for and evaluates food/drug interactions. Assesses the effectiveness and appropriateness of nutrition and food therapies. Assists with preparation of nutritional analysis of general and therapeutic cycle menus.5. Observes, documents and reports the patient's responses to treatment, any change in the patient's condition, or revisions of the patient's plan of care to the physician, and/or Assistant Director of Nursing or Director of Nursing.6. Assists with the coordination of the plan of care and maintains continuity of patient care through interdisciplinary coordination with the other healthcare professionals by participating in patient care conferences as needed.7. Develops, prepares and maintains individualized patient care documentation with accuracy, timeliness and in accordance with the Agency's policies and procedures, and regulatory requirements.8. Fulfills the obligation of requested and/or accepted case assignments. Completes other assignments as requested and assigned. Participates in interdisciplinary case conferences and case load reviews to ensure optimal clinical outcomes for assigned patients.9. Escalates patient care concerns in a timely manner to Assistant Director of Nursing or Director of Nursing to prevent patient decline and potential re-hospitalization.10. Participates in Quality Assurance and Performance Improvement/(QAPI) program. Conducts field evaluations of registered dietitians in accordance with regulatory requirements.11. Working conditions include: (a) Daily home visits that require walking, driving or use of public transportation between visits; (b) May be required to climb stairs to make home visits and communicate with the office regularly before and at the end of the day and possibly between patient visits; (c) Travel between/to other boroughs as needed; and (d) Required to carry an agency issued laptop, clinical bag and cell phone.Minimum QualificationsQUALIFICATIONS FOR THE JOB
  • CERTIFICATION(S)/LICENSE(S):New York State License as a Registered Dietician; Current registration with NYS Department of Education in Nutrition; and Current certification in Basic Life Support procedures including CPR
  • EDUCATIONAL LEVEL:Bachelor's Degree or higher in dietetics or related program approved by the Academy of Nutrition and Dietetics.
  • KNOWLEDGEABLE IN:All aspects of a qualified Registered Dietician skill set. Some knowledge of Home Care regulations preferable but not mandatory.
  • YEARS OF EXPERIENCE:One year of post-licensure experience as a Registered Dietician.
