Job Description:
Description $2,000 sign-on bonus. This bonus is paid out $1,000 at 6-months and $1,000 at 12-months. Employee must be up to date with all trainings and in good standing. East End Disability Associates Inc. is a non-profit organization that provides services and support for individuals with intellectual and developmental disabilities (IDD). EEDA is hiring full-time Direct Support Professionals (DSPs) for our Day Habilitation Programs. EEDA's Day Hab DSP provides the people supported the opportunity to increase independent living, socialization, and vocational skills through participation in person centered planned activities in the community and at the program site. We have locations in Riverhead and Calverton. Shift: Monday-Friday, 8am-4pm Hourly Rate: $17.50 at hiring. $18.50 includes additional certifications obtained once employed. Benefits:

Essential Duties and Responsibilities:
  • Supervision of the people supported.
  • Implementation of Program Habilitation Plans.
  • Assist in planning activities.
  • Assist the people supported when needed. Including areas of self-care, independent living, and community integration.
  • Transportation/driving agency vehicles.
  • Attend all mandatory trainings sessions including new hire orientation, Defensive Driving, First Aid, CPR, program specific trainings, and annual trainings.
  • Obtain SCIP (Strategies for Crisis Intervention and Prevention) and AMAP (Approved Medication Administration Personnel).
  • Complete all necessary documentation, which includes daily Day Service Outcomes Data Collection and Activities Record; Day Hab Monthly Summaries including distribution; Day Service Assessments; Medication Records (if AMAP); Incident Reports, Seizure Reports and Behavioral Records when needed; and any necessary correspondence.
  • Correspond as needed with other program staff, family members, consultant staff (e.g., BISs, RNs, Nutritionists, etc.).
  • Any additional responsibilities for the successful operation of the program.

  • Valid NYS driver's license with an acceptable driving record.
  • High School Diploma.
