Description OPEN UNTIL FILLEDJOB SUMMARY:The Direct Support Professional located at the Geriatric Transition Center will provide direct services to individuals with mental, behavioral, and substance abuse issues within a Transition Center unit that provides residential stabilization as well as medically-managed services. Provides direct care support and instruction, as needed, and assist treatment team members in implementing individualized recovery plans. Completes household duties to ensure cleanliness, safety, and compliance with licensure regulations.ESSENTIAL FUNCTIONS:
Provide individualized and group services to adults meeting criteria for the Geriatric Transition Center in order to promote recovery and meet the program goal of reducing reliance on more restrictive inpatient services. Assist in implementing Individualized Recovery Plans, including support, assistance with independent living skills, social skills, personal hygiene, household skills, cooking, nutrition and meal planning education and assistance, and transportation to recreational groups and activities.Ensure a clean and safe environment by performing such task as fire drills, fire extinguisher checks, house cleaning, grocery shopping, meal planning, and preparation.Participate in the activities of a self-directed team, and as needed, assigned, or scheduled, to attend meetings, and trainings events.Maintain, administer, and document medications in accordance with written physician's orders and regulatory standards, and maintain current certification in the Agency's approved medication training program. Observe persons for potential side effects of medications, and notify appropriate staff as necessary. Administer basic first aid as needed and maintain current certification in CPR, First Aid, and other required training.Appropriately intervene in crisis situations and emergencies, making referrals and seeking consultation from other team members as needed.Participate with team members in the formulation of person-centered recovery planning. This may also include developing goals, objectives, plans, and data sheets in order to provide accurate and comprehensive documentation.OTHER DUTIES: Perform such other duties as assigned by supervisor.QUALIFICATIONS:•Ability to interact with people in a supportive and therapeutic manner•Knowledge of the nature of severe mental illness and substance use disorders.•Good oral and written communication skills•Ability to assist in developing and implementing service plans•Skills in data collection•Ability to work as part of a team•Must be able to work independently and with supervision available at broadly-spaced intervals•Ability to pass behavioral modification, medication, and other required trainings, with or without accommodation.Minimum Requirements EXPERIENCE/EDUCATION REQUIRED:High school or equivalent.Valid driver's license with a safe driving record.
