Description Position: Direct Support Professional (Individualized Services)Supervised by: Individualized Services CoordinatorSchedule: TBDHourly Rate: $19.25/hr.General Description of Position:The person in this position will be responsible for training, supervising, and supportingindividuals with disabilities, both at their home and in the community, specific to theindividuals' needs. This position will function according to the policies established byTurning Leaf Agency, with the goal to maximize the potential of the individuals in theprogram.Essential Responsibilities:1. Treat all individuals in an age-appropriate manner, with respect and dignity, andencourage opportunities that promote choices.2. Train, supervise and support participants in community, vocational, social/leisure,recreational and other identified skill areas; ensuring quality work and meetingestablished production standards set by employer(s), as appropriate.3. Maintain a professional environment during work hours.4. Keep the administration informed of any changes in established policies andprocedures or the need to modify any area of the operation/activity and have opencommunication with the individual's residential staff/family.5. Provide the individual with meaningful, goal related supports6. Maintain a professional and healthy relationship with the individuals family withinthe home you work7. Document all services provided to the individual in multiple platforms ( Therap andEVV)Other Related Responsibilities1. Attend all agency staff meetings2. Keep informed and implement current trends and methods of working withindividuals with disabilities if changes are made to DDS documents3. Maintain good daily attendance; Be cooperative, versatile, and work as a teammember4. Maintain a positive attitude; Ability to follow oral and written instructions5. Awareness of and sensitivity to cultural and socioeconomic characteristics ofpersons and population served.6. Be able to respond to emergency situations such as hospital visits and reaching outto guardians, group homes and other family contacts followed by submittingnecessary incident reports in a timely/appropriate manner; Committing your timeafter hours, if necessary, per individual needs in case of an emergency for theindividuals you are assigned to.7. If an agency vehicle is not available for transporting our individuals served, you willbe asked to transport the individual in your personal vehicle for their day-to-dayservices (Mileage will be reimbursed)Minimum Education Required: High School Diploma/GED and 2 years ofexperience with the DDS Population.Required Knowledge Skills and Abilities1. Must complete the Agency's Employee Training.2. Certification and Training, including but not limited to: CPR, First Aid, PMT,Medication Certification etc.3. Must be able to become DDS Medication Certified within the first three months ofhire.Additional Requirements1. Must have reliable transportation2. Must have a valid State of Connecticut Driver's License and maintain currentinsurance and registration.3. Must be able to meet physical demands of the job. These include but are not limitedto ability to lift minimum of 50 pounds for assisting participants in personal careneeds and/or the purpose of performing daily job duties, ability to twist, bend andreach immediately to address medical and behavioral incidents, ability to enter andexit agency vehicle at numerous stops for a minimum of one hour and the ability todrive agency vehicle for a minimum of 30 minutes without stops.Note: This job description covers the most significant duties but does not include otherassignments, related duties, or temporary assignments.Position RequirementsFull-Time/Part-Time Full-Time and/or Part-Time Rate of Pay $19.25/hr Position Direct Support Professional Location New London County EOE Statement We are an equal employment opportunity employer.
