Community Health Action of Staten Island (CHASI) drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities - people with low or no income, low-income people with chronic illnesses, people with criminal justice involvement, people who use drugs, domestic violence survivors, people of color, and the LGBTQ community. POSITION SUMMARY:The Director of Care Management is the lead manager and administrator for the Medicaid funded Health Home program. They collaborate with all departments to develop, deliver, and evaluate program services and oversee all operations. The Director will directly supervise the Unit Supervisors, Navigators, and Outreach & Intake operations. This position ensures that core services are provided to clients of the Health Home Medicaid Care Management Program in accordance with New York State Department of Health requirements as well as agency policies, procedures, and quality standards. The Director is a participating member of the agency's Management Team and various workgroups to support the growth and development of the agency. DUTIES & RESPONSIBILITIES: Personnel Management :Provide direct supervision to the program's management team, some direct service staff, and responsible for managing program's outreach process.Oversee and support the processes for recruitment, orientation, training, evaluation, and performance improvement.Develop training plans and learning opportunities for staff, including the provision of and/or coordination for internal training and outsourced instruction.Build a positive staff culture. Provide guidance, coaching, support, and feedback to staff.Establish objective performance measures in concert with those set within CHASI to ensure consistent, high-quality evaluation and goal setting for all employees in the department. Instill a sense of accountability among team members by modeling focused oversight of individual and organizational performance standards. Planning, Fiscal, Compliance : Develop tools and processes to ensure that programs meet their established deliverables and outcome goals; identify areas of risk and weakness; create action plans to address areas of concern.Implement and maintain an aggressive plan to achieve census and deliverable targets. Analyze program performance and forecast future demand and resource requirements to ensure effective program operations.Assist with planning and growth processes to ensure program sustainability. Plan and integrate new programming into program areas for additional funding, other CHASI programming and the parent company.Oversee the program's spending in a fiscally responsible manner, keeping spending within the confines of the programmatic and departmental budgets. Ensure that program activities operate within the policies and procedures of the organization and funders and are compliant with relevant regulatory agencies.Develop quality assurance, continuous quality improvement, and outcome evaluation processes and implement these within the department. Community Engagement :Oversee community engagement including internal and outreach activities geared toward: improving communities' and community members' awareness of health and other concerns and engaging community members in utilization of CHASI's services. Represent CHASI at meetings with partner agencies, funders, and others with clarity of agency mission, departmental programming, and social agenda in a professional and respectable manner.Liaise with coalitions, partner agencies and funders to acquire relevant programmatic information as well as to promote and represent CHASI.Develop and maintain community relationships and referral arrangements with community agencies and hospitals.Provide crisis intervention services as needed. Reporting :Write monthly and as-needed reports for submission to funders.Oversee the preparations and response planning for site visits from funders and regulatory agencies. Team Collaboration :Work collaboratively with the management team to integrate cross departmental activities and functions.Develop and implement strategies that will maximize synergies among program areas.Provide clinical supervision to staff and/or interns across the department's programs.Perform all functions in alignment with CHASI's Mission, Vision, and Core Values. RequirementsQUALIFICATIONS:At least 3 years of experience working in the human services field providing services to individuals experiencing chronic illness including mental health and substance use disorders required.At least 2 years of experience supervising staff and managing programs required. Bachelor's degree required. Master's degree may be considered in lieu of some years of experience listed above.Advanced knowledge of Medicaid/Managed care billing is a plus.Advanced computer skill, specifically Microsoft Office applications, including Excel and PowerPoint as well as internet navigation and ability to use and quickly learn new databases.COMPETENTANCIES:Works independently following receipt of instructions on a task, demonstrating the ability to understand and execute the task with minimal supervision.
