Position may require occasional evening or weekend work.FUNCTIONThis position is responsible for the direction of the Case Management department within the Health and Wellnes Hub, including directing the centralization of Case Management,program redesign and implementation. This role will focus on creating policies and systems that support Case Managers in facilitating client's growth towards stable housing. This role will manage all Case Management-related tools, oversee the contracts that fund Case Management, benefit programs related to Case Management, and provide supervisory support to Supervising/Coordinating Case Managers.TOP RESPONSIBILITIESProgram Development, Implementation, Compliance & Evaluation:
  • Support program design in coordination with the Executive team, Ali's Place Drop-In, and housing leadership team, and appropriate, relevant programs in response to the Ali Forney Center's Mission.
  • Oversee and ensure program data is submitted within a timely manner to funding agencies (DYCD, HUD, SAMHSA, etc)
  • Develop, implement, and evaluate case management design to meet the needs of the people and the program, including;
    • Foster a culture of equity and inclusion
    • Strengthen program success of clients and residents
    • Help maintain consistency in the delivery of services with regard to programmatic policies
  • Stay informed on the trends impacting the NYC landscape around LGBTQ+ youth homelessness, particularly emerging housing opportunities and voucher programs.
Staff Management:
  • Recruit, hire, and train case management team members
  • Provide supervisory support and conduct regular weekly clinical and task supervision with case management team
  • Oversee direct care staff through weekly meetings, training, and direct observation.
  • Be accessible to support the fast-paced work of agency programs, especially during crises, emergencies, or staff shortages.
  • Collaborate with the training Department to develop a standardized training curriculum and professional development for case managers and supervisors.
Client Engagement & Support:
  • Create, implement, and support formal avenues to receive youth feedback and participation in decision-making around department policies, procedures and programming
  • Utilize and coach staff on harm reduction, trauma-informed, and anti-oppressive approaches to client care.
Collaboration, Communication & Partnership:
  • Lead and encourage cross-programmatic care coordination and communication with Ali Forney Center Programs (Health & Wellness, Learning & Life Skills, Housing, Ali's Place Drop-In).
  • Collaborate with community partners, build relationships with peer organizations to share best practices, and coordinate appropriate programming.
  • Collaborate with Youth Advisory Board members to support and ensure clients' needs are centered.
  • Manage Resource Directories and other case management tools.
  • Provide daytime supervisory support and conduct regular supervision with Supervising Case Managers.
  • Attend and support the facilitation of weekly staff meetings including Housing and Drop-In Treatment Planning, Case Management Team Meetings, Placement Meetings, Community Accountability Meetings, Staff Training, etc.
EDUCATION REQUIREMENTS [ ] High School[ ] Vocational Training[ ] Undergraduate Degree[X] Masters DegreeLMSW required and/ or LCSW obtained within 1 year of hire REQUIRED SKILLS Highly organized, ability to multitask and manage different priorities under pressure, ability to work independently and with others, effective verbal and written communication. Effective computer skills and proficiency in Microsoft Office and Google Suite. Extensive knowledge of service delivery systems for homeless youth. Previous supervisory experience and 4 years of case management experience. Previous experience working with LGBTQ+ folks. Experience working with the adolescent/young adult population.Knowledge of psychosocial needs of LGBTQ/homeless population.Experience, competency, and comfort working with transgender individuals.PREFERRED QUALITIESAbility and willingness to work flexible hours, weekends and holidays.
