This role offers the opportunity to drive quality patient outcomes while ensuring cost-effective care across the continuum. Key Responsibilities: Provide strategic direction for clinical care coordination, discharge planning, and case management services. Collaborate with interdisciplinary teams to improve care transitions, reduce readmissions, and enhance throughput. Manage departmental resources, budgeting, and quality control while ensuring regulatory compliance. Serve as a resource to senior leadership, staff, and interdisciplinary teams, promoting innovation and evidence-based practices. Facilitate performance improvement processes and align departmental operations with organizational goals. Qualifications: Experience: 5 years in case management, discharge planning, or cost/quality management. 3 years in a hospital clinical role. Supervisory/management experience required. BSN required; Master's degree preferred. Active Ohio RN License (Required). BLS (AHA) Certification (Required). Why Join? Location: Lima, OH. Schedule: 13-week contract with a full-time commitment.
