Join us in making a difference without sacrificing your well-being!The Director of Clinical Services (DCS) offers both strategic and day-to-day supervision in the field of home healthcare, ensuring compliance with regulations, fostering collaboration among different healthcare disciplines, and fostering a patient experience and clinical outcomes that surpass expectations. The primary duty of a Director of Clinical Services at Pinnacle Home Care is to ensure the motivation, dedication to clinical excellence, focus, and effectiveness of their clinical staff and teams. As a Pinnacle leader, it is expected that this will be accomplished by a laser focus to teach, develop, motivate, and MAKE THEIR TEAMS BETTER every day. This role also involves staffing and staff management, mentorship, advocacy for patients, and adherence to established policies and professional standards. The position involves supervising Clinical Managers, monitoring performance, and supporting innovative initiatives. As the designated Clinical Manager according to CMS, the role oversees patient care services, assigns personnel, and contributes to care plan development.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

  • Provide strategic direction and oversight to the clinical team, including nurses, therapists, and other healthcare professionals.
  • Implement quality assurance programs to monitor and improve the delivery of patient care, ensuring adherence to industry best practices and regulatory requirements.
  • Ensure compliance with all federal, state, and local regulations governing home healthcare services, including Medicare regulations.
  • To ensure coordinated and comprehensive patient care, foster collaboration among healthcare professionals, including physicians, therapists, social workers, and support staff.
  • Execute the screening and interviewing process of new organization personnel and make recommendations for the employment of individuals. Ensures success of New Clinician Orientation Experience
  • Train and mentor clinical staff, promoting professional growth and development. Conduct performance evaluations and provide constructive feedback.
  • Oversee the accurate and timely completion of patient assessments, care plans, and documentation, in accordance with established guidelines.
  • Act as an advocate for patients and their families, ensuring their needs and concerns are addressed promptly.
  • Identify opportunities for process improvement, cost reduction, and enhanced patient care. Implement Pinnacle's SOP and policies to achieve operational excellence.
  • Maintain emergency preparedness plans to ensure the safety and well-being of patients and staff during crises.
  • Support case conference meetings with organization personnel to facilitate coordination of care.
  • Conduct quarterly record reviews and communicate findings and recommendations to immediate supervisor and appropriate organization personnel.
  • Comply with accepted professional standards and principles.
  • Lead team by Pinnacle Mission, Vision, and Values.
  • Supervise, coach, and mentor Clinical Managers and Clinical Assistants in compliance with Standard Operating Procedures.
  • Promote Case Management with Field Clinicians.
  • Ensure continued education occurs in the field and during planned 1:1 meetings with clinicians consistently.
  • Monitor SHP reports for the Agency's current performance measures, outcome measures, VBP, and HHCAHPS scores and implement Performance Improvement Plans to exceed state and National Averages.
  • Promote the use of patient portals, physician portals, and Synzi.
  • Promote Telehealth visits to support appropriate visit utilization.
  • Function as the CMS Clinical Manager: Will always be available during operating hours and will provide oversight of all patient care services and personnel including making patient and personnel assignments, coordinating patient care, coordinating referrals, assuring patient needs are continually assessed, and assuring the development, implementation, and updates of the individualized plan of care.

POSITION QUALIFICATIONS

  • Registered nurse with current licensure to practice professional nursing in the state.
  • Graduate of National League for Nursing accredited school of nursing.
  • Two years of management or supervisory experience in home health care.
  • Complies with accepted professional standards and practice.
  • Complies and maintains current CPR certification.
  • Ability to deal effectively with high levels of stress.
  • Strong knowledge of Medicare regulations.
  • Excellent leadership, communication, and interpersonal skills.
  • Proficiency in healthcare software and electronic medical records.
  • Ability to work collaboratively in a team environment.
  • Knowledge of governmental regulations and ACHC standards.
