OverviewThe Clinical Director of Counseling and Support Services is responsible for overseeing the counseling and volunteer operations of the hospice program. They are responsible for ensuring that patients receive high-quality emotional, spiritual and bereavement support in accordance with hospiceregulations. This role is responsible for the volunteer services program and ensuring delivery of a quality volunteer program that supports the hospice program, patients, and families. They act as a champion of education, training, and delivery of quality care and are a resource that provides guidance and support to their respective teams. They are a critical part hospice orientation, and their teams ongoing education, growth and retention. In this role, they work side-by-side with the clinical and operational team to promote the counseling and volunteer teams roles. Travel regularly to support regions is required. This role requires flexibility in time worked to be available for their preceptors, social workers, spiritual care coordinators and volunteer manager.Compensation:$109,900.00 - $146,500.00 AnnualLeadership and Management:Provide leadership and direction to the social work, spiritual care, bereavement and volunteer teams.Ensure effective communication between IDG teams.Develop and implement standard operating processes related to areas responsible for.Analyzes and resolves complex SDOH case management problems in collaboration with hospice leaders.Ensures that the highest quality of patient emotional, spiritual and bereavement care is provided to hospice patients and their caregivers.Oversees coordination/management of schedules to meet operational needs for areas of responsibility.Performs all duties inherent in a director level position, including, hiring, training, supervising, evaluating, recommending salary actions and when necessary, terminating assigned staff in consultation with Senior Leadership and in accordance with Agency policies and proceduresParticipates in the development of the counseling and volunteer program budget and is responsible for financial stewardship of that budget.Clinical Oversight:Directs and supervises delivery of effective and exemplary supportive care provided to hospice patients and families.Ensure compliance with all relevant regulations, accreditation standards, and hospice policies for areas managing.Collaborates with clinical specialties, other Clinical Directors and leaders in the implementation of patient care.Conduct regular reviews of clinical documentation for accuracy and completeness.Quality Assurance:Monitor quality of care and patient outcomes, implementing improvements as necessary.Conduct case reviews and provide guidance to staff on complex patient situations.Participate in quality improvement initiatives and ensure adherence to best practices.Responsible for ensuring compliance with NY State Department of Health, CHAP and CMS rules and regulations in the delivery of hospice care.Staff Development:Provide training, mentorship, and support to volunteer manager, social work team, spiritual care coordinators and bereavement.Conduct performance evaluations and facilitate ongoing professional development.Collaboration:Collaborate with other departments and healthcare providers to ensure seamless patient care.Serve as a resource for family members and caregivers, addressing concerns and providing guidance.On-Call Responsibilities:Respond to emergencies and make clinical decisions in high-pressure situations.Community Engagement:Represent the hospice in community outreach and education efforts.Build relationships with local healthcare providers and organizations to enhance service delivery.QualificationsEducation:Master's Degree from accredited Theological School or Master's Degree in Social Work requiredWork Experience:Minimum of five (5) years' experience in health care with a minimum of three (3) years of focus in Hospice requiredStrong clinical experience in a hospice setting, including leadership experience requiredStrong clinical, communication and interpersonal skills requiredStrong process operation, efficiency, collaboration, openness and results orientation requiredExcellent critical thinking and problem-solving abilities requiredCompassionate and empathetic approach to patient care requiredPolished facilitation and presentation skills requiredKnowledge of HCHB operating system for hospice preferred#J-18808-Ljbffr
