Every employee takes a person-centered approach that exemplifies the ICARE values (Integrity, Compassion, Accountability, Respect, and Excellence) through empathic communication and partnerships between all persons.Job DescriptionSUMMARY OF ESSENTIAL DUTIES AND RESPONSIBILITIES:Patients transitioning out of the acute care setting-including those discharged to home, shelter, or skilled nursing settings, among others-remain among the most vulnerable to experiencing fragmentation and gaps in care, increasing risk for preventable adverse outcomes and downstream costs of care. Safety net hospitals and health systems face unique challenges in delivering effective transitional care in the post-acute period, serving a patient population with an overall higher risk of social admissions, readmissions, and greater unmet behavioral health, economic, and housing needs. Meeting these needs requires strong collaboration, coordination, integration, and management of care planning and delivery across disciplines within and beyond the hospital setting and outside the health care sector.The Director of ED Care Transitions will support systemwide post-ED strategy, operations, implementation and improvement for patients moving within acute facilities to inpatient or community-based settings. Partnering with hospital and Central Office leadership, ED Care Management (ED CM), ED Social work, inpatient Transitional Care Management (TCM) staff, and community-based staff, the Director will support staff training, workflow development and implementation, and continuous improvement to ensure ED based programming's program effectiveness. As the primary central support staff for facility-based ED care management work, the Director will also oversee coordination with clinical (especially inpatient TCM teams) and business stakeholders and other internal partners (e.g., Epic resources) for all program modifications, and development and refinement of performance management and program evaluation tools. With support from the Senior Director of ED Access and Equity, the Director will identify gaps, review key performance metrics, promote program development and strengthen stakeholder engagement for development of the program.The responsibilities of the Director will include but are not limited to:•Coordinate system ED CM committee meetings and inpatient-ED TCM subcommittee meetings, including with facility TCM and ED leadership and key central office stakeholders.•Collaborate closely with facility CM program directors to support roll-out of a standardized ED CM program, ensure integration with other CM programs, and ensure that sites meet key program outcomes.•Regularly visit with all system acute care hospitals to identify and support the needs of facility, ED leadership, CM leadership and other staff.•Coordinate and lead monthly check-in meetings with sites to facilitate ED CM program implementation (including but not limited to participation in ED High Utilizer committee meetings in an advisory role), review data and program outcomes, guide operational workflows, and address barriers related to ED CM standardization.•Develop standardized workflows and templates for Acute Care Planning activities for ED patients (especially those identified as ED High Utilizers, PEH w/ SMI, TCM-enrolled ED patients, patients within 90 days post-IP discharge etc.) to ensure broad construction of quality acute care plans systemwide.•Collaborate with Central Office and facility leadership to support design and implementation of system strategy to increase linkage of ED and other acute care patients with appropriate and timely primary care and specialty follow-up prior to discharge. Including but not limited to assisting with EPIC innovations, referral guidelines, socialization of initiatives at the facility level, data review, and continuous QI.•Coordinate and collaborate with local and systemwide staff to help implement and drive Social Admission Prevention and TCM Readmission Prevention strategic initiatives.•Liaise with facility leadership to coordinate ED CM realignment and hiring of local ED CM directors and provide close support as new staff transition into this role.•Develop standardized operational guides, job descriptions, policies and procedures that provide a comprehensive overview of expected standard work, roles of different staff members, and support facilities to adopt policies and procedures.•Develop and improve a systemwide training program, collaborating with internal and external stakeholders to design an orientation program, training curriculum and associated materials, and a continued professional development program, as well as identifying training gaps and developing innovative models of delivering training across the system.•Coordinate and collaborate with key Central Office staff including across the Office of Ambulatory Care, Office of Patient Centered Care, Office of Managed Care and Patient Growth, Community Care, Social Work and the ACO, to develop new care pathways that adapt the CM model to meet the needs specific patient populations, including by certain conditions or social needs.•Develop, maintain and improve ED CM reports and dashboards to monitor program implementation, review key process metrics and program outcomes and drive quality improvement.•Collaborate with the Epic team to optimize the ED CM build, identifying opportunities and tools to improve workflows, documentation and performance management.•Lead development of process metric reports with Data and Analytics team and integrate into Epic reports and dashboards.•Participate in meetings with the Central Office and facility teams to conduct review of ED CM.•Participating in ongoing education, specialized training and research to learn and maintain skills needed for high quality care management.•Perform other duties as assigned.Minimum Qualifications1. A Master's Degree from an accredited college or university in Public Health, Public, Hospital or BusinessAdministration, Health Care Specialization, Engineering, Social Services or a related discipline; and four (4)years of progressively responsible experience relating to health care program planning, research, design,operations, evaluation and analysis, one (1) year of which must have been in supervision or planning and/oranalysis; or2. A Baccalaureate Degree from an accredited college or university in disciplines, as listed in #1 above; and five(5) years of progressively, responsible experience relating to health care program planning, research, design,operations, evaluation and analysis, one (1) year of which must have been in supervision or planning and/oranalysis; or3. A satisfactorily equivalent combination of education and experience, in which 30 graduate-level semestercredits from an accredited college or university can be substituted for one year of experience. However, allapplicants must have at least a Bachelor's Degree.Department Preferences•Master's degree with preference in nursing, health administration, public health, or health care management or related field.•At least 5 years of full-time clinical, operational, or administrative experience in a hospital inpatient or outpatient department.•At least 2 years of full-time experience in transitional care programming or related activities within a health care delivery organization or health insurance entity.•Project Management: Independently develops and/or uses an organized approach to managing multiple deadlines in parallel across different initiatives and teams.•Workflow Operations: Analyzes and breaks down complex operations into discrete process steps and branches; uses process mapping / swimlane diagrams to describe processes visually.•EPIC or electronic medical record•Quality Improvement: Uses core quality improvement tools to guide process improvement, including outcome, process, and balancing measures; driver diagrams; and run charts.•Analytics, modeling, visualization: Manipulates and analyzes large data sets, including for operational or financial planning purposes. Identifies data needs and modeling assumptions Visualizes data in tables and charts for lay interpretability.•Interpersonal teams: Works well with individuals of different personality types, work styles, and backgrounds. Comfortable leading groups or teams, including with more senior employees•Strong written and verbal communication skills and comfort managing complex teams.•High proficiency in managing in-person and virtual meetings, including goal-direction, attention to time management, and ability to encourage collective participation.•Ability to identify workflow challenges, independently explore and identify root causes, and work through steps in order to develop solutions.•High degree of organization and ability to manage multiple projects concurrently, including under tight timelines.•High proficiency in communicating workflows through visual flow diagrams and process mapping techniques.•Comfort and independence communicating across departments or disciplines, including in an instructional or training capacity. Uses an empathic and non-punitive approach, eliciting new information and challenges.•Strong proficiency in data analytics and interpretation.
