Job Description:
Statement of Job: The Solana Garden Director shall be responsible for the total care of the Memory Care residents. Duties include but are not limited to the following: Care Management
  • Conduct initial Needs and Risk assessments prior to move-in
  • Conduct reassessments quarterly and with change of condition
  • Communicate with ED and family when a level of care change is indicated
  • Track monthly weights, vital signs on all residents; intervene as required
  • Oversee all dementia programs by daily rounds and clinical supervision
  • Develop alliances with Third Party Providers and meet with them regularly to discuss at-risk residents and those on case load
  • Attend to all residents with emergency medical needs and train staff to follow emergency medical protocols; audit compliance with emergency alert system
  • Monitor the recording of all relevant information to Communication Log and resident record, to include Monthly Summaries when required
  • Maintain resident files in compliance with state guidelines; audit/thin resident records according to established schedule
  • Medication Management
  • Oversee pharmacy set-up and relationship for all new residents
  • Train new Med Techs as required and complete initial competency assessment
  • Complete competency assessment annually for all working Med Techs
  • Audit MARs monthly, to include Narcotic Count, and maintain Medication Variance Log
  • Conduct bi-monthly Med Tech meetings
  • Report all medication errors to corporate or state personnel as appropriate
  • Respond to all Pharmacy QA visits and reports
  • Staff Management
  • Interview and hire Resident Care staff
  • Orient new staff and assign them to Mentors
  • Complete initial competency assessment of all new Resident Care staff
  • Establish and monitor staff schedule and care assignments for staff, including tardiness and absences
  • Monitor staff documentation and feedback on all residents, especially those with changes of condition
  • Hold bi-monthly staff meetings to include at least one formal in-service training session per month
  • Document staff compliance with in-service training requirements and follow-up with staff as necessary
  • Provide formal evaluations of staff as scheduled and informal counseling as needed
  • Maintain staff appreciation for all Resident Care staff
  • Business Management
  • Control labor hours according to approved budget
  • Utilize departmental spend-down sheet to control costs
  • Assist in marketing community by providing tours as requested
  • Risk Management, Quality Assurance and Compliance
  • Conduct or provide proper body mechanics in-service at least twice yearly
  • Provide initial training to all staff on infection control and emergency procedures
  • Monitor staff compliance with CPR and First Aid training
  • Participate in monthly safety committee meetings
  • Investigate all resident injuries and develop appropriate interventions
  • Follow risk intervention protocols (Shared Risk Agreements)
  • Investigate all employee injuries and manage staff released to light duty
  • Conduct audits of emergency alert system
  • Maintain OSHA guidelines as they affect Resident Care department
  • Investigate any allegation of resident abuse in compliance with state and corporate standards
  • Track clinical indicators and report monthly for Quality Assurance Program
  • Respond to any state survey in a timely manner
  • Resident and Family Relations
  • Respond to resident and family concerns with kindness and compassion
  • Maintain frequent in-person and phone contact with residents and families
  • Train Resident Care staff to focus on customer service
  • Demonstrate good customer service and the Arbor Attitude at all times
