Crane Home Care- Director of Nursing Th Director of Nursing is responsible for the direction, coordination, and direct supervision of the agency's paraprofessional staff. This position provides in-field professional supervision and on the job training to home health aides, personal care aides and all nursing staff. The Director of Nursing is also to assist in the agency's continuing education/in-service, orientation and competency testing of all clinical staff. The Director of Nursing is also responsible for creating new revenue streams that best benefits in the organization. This position is full time and will require dedication and flexibility. - Requirements:
- Current unrestricted RN license In New York State.
- Bachelor's degree in Nursing (preferred)
- 1 year of Licensed Home Care Services Agencies (LHCSA) experience
- 1 year of Home Health Operations Management experience
- Understands regulatory compliance with state and federal regulations
- Knowledge of DOH regulations
- Adhere to HIPPA and maintain client confidentiality
- Possess exceptional nursing assessment skills
- Policy Development is a plus
- Ability to travel daily

- Duties/ Responsibilities:
- Supervise and train other nursing professionals and caregivers.
- Communicates with referrers/discharge planners to review eligibility for agency services and develop and coordinate the plan of care.
- Provides assessment of newly referred patients, assesses health needs and eligibility for home care.
- Identifies the appropriate level of care required by the patient.
- Initiates the Medical Orders and written reports to the patient's physician as necessary.
- Reviews reports from all involved disciplines, re-evaluating needs and supervise all disciplines in the plan of care.
- Documents accurately and completely in the patient's clinical record.
- Becomes familiar with all resources necessary to provide a complete managed program for the patient.
- Participates in the agency's CQI program, annual policy/procedure review as required.
- Performs Quality improvement duties, completes audits and evaluates results with DON, recommendations for quality improvement.
- Conducts aide's competency reviews.
- Reviews and responds to complaints, grievances, and incidents.
- Interprets and reinforces agency policy and procedures to professional and paraprofessional staff.
- Business Development.
- Performs other duties as required and assigned by CEO/Owner.
