Director of Population Health & Health Equity

Salary: $120k – $140k

Nassau University Medical Center (NUMC) is a leading academic medical center located in East Meadow, New York. As the only public hospital on Long Island, we are committed to providing high-quality, compassionate care to our diverse community. With over 500 beds and 3,000 employees, we offer a wide range of inpatient and outpatient services across numerous medical specialties.

We are seeking a visionary and experienced Director of Population Health and Health Equity to lead our organization's efforts in improving community health outcomes and reducing health disparities. The ideal candidate will have a strong background in public health, healthcare administration, and social determinants of health.

In this role, you will:

  • Develop and implement comprehensive strategies to address population health challenges and health inequities.
  • Collaborate with community partners, healthcare providers, and policymakers to create and sustain health improvement initiatives.
  • Analyze health data to identify trends, disparities, and areas for intervention.
  • Design and oversee programs targeting specific health issues and at-risk populations.
  • Lead a team of health professionals and coordinate cross-functional efforts.
  • Secure funding through grants and partnerships to support health equity initiatives.
  • Advocate for policies that promote health equity and address social determinants of health.
  • Evaluate the effectiveness of programs and initiatives, making data-driven recommendations for improvement.

We require:

  • Master's degree in Public Health, Healthcare Administration, or related field
  • 7+ years of experience in population health management or related field, with at least 3 years in a leadership role
  • Demonstrated commitment to health equity and experience working with diverse communities
  • Strong understanding of social determinants of health and their impact on population health outcomes
  • Excellent analytical skills and experience with health data analysis and interpretation
  • Proven track record in program development, implementation, and evaluation
  • Outstanding communication and interpersonal skills, with the ability to build relationships across diverse stakeholder groups
  • Experience in grant writing and managing budgets
