The Director of Referral Management will oversee the management of patient referrals from Mass General Brigham (MGB) capacity programs, ensuring efficient coordination and transition of care across the Marquis network. This role will focus on optimizing referral processes, building strong relationships with MGB partners, and managing the flow of patients to ensure seamless care delivery in post-acute settings. The Director will collaborate with clinical teams, case management, and hospital systems to enhance patient outcomes and support capacity management goals.
Responsibilities
- Develop, implement, and oversee referral management strategies for patients transitioning form MGB facilities to post-acute care within Marquis.
- Manage and streamline referral processes, working closely with MGB to reduce delays and enhance patient throughput.
- Monitor referral data, analyze trends, and identify opportunities for improvement in capacity management and patient care transitions.
- Collaborate with clinical and administrative teams to support the referral of medically complex patients and ensure alignment with patient care goals.
- Establish and maintain communication pathways between MGB and Marquis facilities to support efficient care coordination.
- Lead quality initiatives related to referral management, ensuring that goals and metrics are achieved.
- Maintain and grow relationships with key stakeholders at MGB and other referral sources.
- Lead a team responsible for managing the referral process, providing leadership and support.
- Collaborate with the Vice President of Strategic Development on initiatives to grow referral volumes and improve capacity management.
Qualifications
- Bachelor's degree in healthcare administration, business, or a related field; master's degree preferred.
- 5-7 years of experience in referral management, healthcare operations, or case management.
- Proven experience in managing patient referrals and collaborating with hospital systems.
- Strong leadership, communication, and project management skills.
- Ability to work effectively with cross-functional teams to drive process improvements.
- Familiarity with patient transitions between hospital and post-acute care settings.
- Experience with healthcare data analytics and performance reporting.
