Establishes objectives for the department and the organizational structure for meeting those objectives. When you come to work at an Ebenezer community, we strive to support your growth, honor your strengths and empower you to make a difference in the lives of others.Directs department activities to meet State and Federal regulations •Develops and maintains policies and procedures •Develop and operate within department budget. •Assigns caseload and other responsibilities to social workers in department •Evaluates work performance of departmental employees •Supervises Social Workers and Interns as appropriate •Interviews and hires Social Service staff •Maintains departmental records •Participates in continuing education •Maintains Social Work Licensure •Assures residents are informed of rights and responsibilities, according to State and Federal regulations. •Oversees admission process •Participates and assists with Vulnerable Adult reporting and investigationRepresents Social Service perspective in committee meetings dealing with resident care or customer satisfaction. •Provides orientation and in-service training for staff re: Resident Rights, Vulnerable Adults, and Advance Directives as appropriate •Participates and assists with corporate and facility/community events. •Participates in QA meetings and provides necessary reports for review •Participates in Truth point Survey process as appropriate •Assists with investigation and follow up regarding grievancesProvides pre-admission counseling, information, referral, and tours •Facilitates the intake process •Facilitates PAS screening and Level II as needed •Coordinates admission process with resident and resident representative, community providers and staff as appropriate •Assures completion of admission paperwork •Informs residents of financial responsibilities, payment options and agency contacts according to the federal regulations •Builds and maintains relationships with referral sources; and assists with marketing and outreach as appropriate. •Coordinates room change process as appropriate •Verifies necessary payer source(s) as appropriateMakes referrals for psychology services, hospice and develops behavioral contracts as appropriate •Assists residents and resident's representative with financial questions and directs to the business office as appropriate •Refers resident's representatives to support services and/or educational information available in the community •Facilitates and coordinates Family Council per facility policyCoordinates care conference process •Completes psychosocial history and other related Social Service forms and documentation •Coordinates and facilitates care conferences •Assesses psychosocial changes and identifies strategies to address those changes •Maintains departmental records and documents as indicated and required •Completes MDS and associated assessments per regulations •Assists resident and resident's representative in changing primary care providers as appropriate •Advocates for resident's rights and needs •Assists with Advance Care planning conversations and documentationCoordinates Resident and Family Council as appropriate •Advocates for residents as appropriate •Provides information and referral to community service providersFacilitates discharge plans for resident •Arranges in home and community-based services •Ensures that residents and their representatives are informed of the IDT's recommendations for discharge •Educates residents and their representatives about resources available in the community •Provides referrals to other agencies as needed, assists in Vulnerable Adult investigations and completes necessary documentation and reports •Completes corresponding reports as appropriate (Missing item, OHFC, MARC, Police Report) Required:
- Bachelor's Degree: List - BSW (from an accredited school of social work)
- Licensed by the State of Minnesota Board of Social Work
- One to Three years previous experience
Preferred:
- Three to Five years previous experience
- Two years experience in LTC
- Supervisory, management and administration skills
- Experience in medical social work
