It provides Independent Living and a comprehensive retirement living option for adults over the age of 62 by offering an Active Adult Community, Assisted Living, Skilled Nursing Care and Memory Care, all centrally located on one campus.

We are currently seeking a Director of Social Services to direct and supervise social service programs.

In this position, you will:

• Evaluate the social needs of each resident admitted and formulate a written plan of care.
• Review and revise care plans.
• Refer residents and families to appropriate community resources.

We require:

• Bachelors' degree in social work; Master's degree preferred
• Experience with Sigmacare and MDS completion
• License/Certification/Registration in accordance with New York state laws and regulations
• Knowledge of Federal, state, and local regulations, particularly Medicare and Medicaid regulations and long-term care operational standards
• Experience in a Skilled Nursing Facility required; assisted living experience preferred
• Minimum of three years of experience as a social worker with the geriatric population preferred
