Recruit Secure is collaborating with a Skilled Nursing Facility (SNF) located in Staten Island, NY that is seeking a dynamic and energetic individual to take on the full-time role of a Director of Social Services. The SNF is a long-term nursing and rehabilitation facility with about 300 beds and is dedicated to providing the highest levels of care and commitment to its residents, their families, and their employees. A competitive salary, excellent benefits package, 401K, Employee recognition programs, and many other perks are offered.Responsibilities:Assists in Hiring and managing and is a resource to the Social Services team.Sets goals for the department and communicates clearly, and meets regularly with the Social services teamAssures that policies and procedures are adhered to for the SNF; makes recommendations, updates, and revisions to policies when and if needed.Develop the Social Services plan of care for residents, identifying goals and approaches and participating in inter-department care planning meetings.Participates in Quality processes of the SNFCounseling residents and family members; facilitate family meetingsComplete Admissions, Psychosocial Assessments and Advanced Directives and assures placement in medical records.Oversees referrals for health services and facilitates the discharge planning process for the departmentCompleting all required documentation for medical records; producing monthly departmental reports for the SNF Administrator.Requirements:Master's degree in Social Work, LMSW preferredMust have at least 4- 5 years experience working as a Licensed Social Worker within a Nursing or Rehabilitation Facility.At least 1 -2 years experience in a Senior or Director roleShould have a good working knowledge of available social services and other community resourcesMust have excellent communication and written skills#IND123
