Job Overview:
The Director of Operations with oversight of PCCH and PCMH at Jordan Valley Community Health Center (JVCHC) is a role that engages and collaborates across the organization. The efforts of the PCHH Initiative are expected to close care gaps for enrollees, improve coordination of care, support all of JVCHC's primary care and behavioral care practice sites to improve the quality of clinician efficiencies and improve patient outcomes. This program is accredited by the PCMH accreditation. This accreditation is designed to increase quality of care, improve patient and clinician experience, and lower costs for the organization by working to meet payor care gap requirements.
Duties:
- Provide organizational training in regard to PCHH and PCMH clinical standards
- Collaborate and develop integrated care pathways between JVCHC's high risk patients and available resources/ service lines
- Coordinate all the PCHH team activities, manages personnel, and develops KPI and goals in alignment with PCHH and PCMH standards, and beyond
- Champion practice transformation and care gap closures with providers and clinical teams
- Stay up to date and knowledgeable of PCMH accreditation and PCHH Initiative changes
- Ensure that all areas of JVCHC are operating in compliance with NCQA and MPCA regarding PCHH and PCMH, including documentation and records per the guidelines
- Act as primary contact and spokesperson with NCQA and MPCA for any requests or auditory actions
- Collaborate with Director of Operations, Quality to ensure decision making, program development and patient care is reviewed, monitored and tracked through data and statistics for care gaps.
- Strategize opportunities to increase enrollment into the PCHH program and for future endeavors to expand care management/ CCM services across JVCHC organization-wide
- Forecast and communicate expansion needs for the PCHH program specifically, and care management collectively
- Collaborate to create, maintain, and utilize data and reporting tools to support requested changes to operations, building the team and program, collaboration with clinical quality teams/ compliance, and to present to the JVCHC executive team
- Maintain direct responsibility and ownership of the PCHH program and PCMH accreditation
- As needed, perform direct care management services dependent on staffing and patient census needs

Required Qualifications:
- Graduation from RN program and current RN license.
- Experience in a leadership position.
Preferred Qualification:
- 5 years of supervisory experience
