Job DescriptionSUMMARY OF DUTIES AND RESPONSIBILITIES:The Director of Quality Management, Clinical Lead will support the Medical Director of the ACO with implementation of clinical quality improvement strategies by coordinating and refining facility-level interventions and working with relevant H+H stakeholders to improve system-wide quality improvement efforts, lead quality assurance auditing process for annual quality reporting requirements of the ACO, and support the advancement of H+H system wide value-based contracting performance by coordinating with vested organizational stakeholders. The Director of Quality Management will report to the ACO's Chief Operating Officer.General tasks and responsibilities will include: Support Medical Director in clinical analyses of ACO patient population, with particular focus on in-depth understanding of high utilizer and vulnerable patients, their points of encounter with health care system and barriers to engagement with care Identifies opportunities to connect ACO patients with appropriate H+H-wide initiatives, such as Health Home and chronic disease interventions Monitor response to hospital admissions and emergency department presentations for ACO patients Assist Medical Director in education and training facility staff for standard workflow for patient notification Support ACO Director of Quality in system-wide chart review quality assurance process for annual quality measure reporting to CMS and other governmental agencies Help coordinate integration of ACO strategy with existing H+H-wide initiatives, such as Patient-Centered Medical Home, population health improvement, and Health Home, prioritizing areas of common measurement and workflows Lead system-wide chart review quality assurance process for annual quality measure reporting to CMS and other governmental agencies Support Medical Director and Chief Operating Officer in development and implementation of innovative patient and provider engagement strategies, clinical operations streamlining strategies, and quality improvement strategies Lead creation of presentations and communications to ACO clinical leadership at facilities Principal contact for ACO clinical leadership at facilities; addressing inquiries, and providing clinical and operations support Support Chief Operating Officer and Medical Director in the presentation of ACO strategies and operations to relevant senior stakeholders (ACO Clinical Leadership, ACO Board of Directors, Office of Population Health, Ambulatory Care Leadership Council, and NYC H+H Senior Leadership) Recommend improvements to existing operations and team processes Assist with federal policy research to inform ACO growth, strategic direction and quality improvement Work with the ACO Director of Performance Analytics to create, validate and implement the ACO's Quality Metric Dashboard in Epic's Healthy Planet. Continue to work on its maintenance and provide training to front-line clinicians in its use Perform other duties as needed
Minimum Qualifications1. Graduation from an approved School of Medicine and completion of an approved residency and Board eligible; and, 2. N.Y. State license; and, 3. Five years of full-time paid experience in a responsible medical and clinical administrative and/or managerial capacity; or, 4. A Master's Degree from an accredited college or university in Hospital Administration, Health Care Planning, Public Health, Administrative Medicine, Business Administration, Public Administration or approved related program; and, 5. Eight years of progressively responsible experience in health and medical services administration, of which at least four years must have been in a managerial or administrative capacity.
Department Preferences A Master's Degree in Art or Science with a specialization in Hospital Administration, Health Care Administration, Administrative Medicine or in Public Health when conferred for a program in hospital administration from an approved college or university. Certification/License/s: RN, PA or higher Knowledge of fundamentals of hospital organization, administration and standards, regulations and laws applicable to hospital operations.
