- Minimum 5 years clinical experience in hospital, home health, or community setting
- Minimum 3 years recent experience in Case Management/UR/Discharge Planning required
- Minimum 2 years of supervisory experience is required
- BSN Degree
Preferred
- Master's Degree preferred, preferably in nursing
Certifications And Licenses Required
- Current and valid license to practice as a Registered Nurse in the state of New Jersey
- Certification in Case Management strongly preferred/required within 1 year of hire
- Basic Life Support (BLS) from the American Heart Association
Scheduling Requirements
- 9am - 5pm each day
- 37.5 hours weekly

Essential Functions
- This role is accountable for the execution, adaptation, and outcomes of case management as documented in the Utilization Management Plan with regard to resources, appropriateness of care, and adherence to a designated length of stay for each patient and in alignment with state and federal guidelines.
- Responsible for the operational oversight of the Case Management Department, including but not limited to clinical resource management, utilization review, discharge planning, patient advocacy, oversight of nursing and social work functions, medical necessity determinations, and concurrent review.
- Ensure that Case Management Department is in compliance with all applicable policies, laws and regulations.
