Serving our Inpatient and Outpatient populations, our Director will lead the way as we continue to enhance our services and provide top notch care to our patients.Highland Hospital is a 261 bed, acute care facility best known for its comprehensive women's services, nationally ranked geriatrics programs, joint replacement program, gastric bypass program and extensive network of primary care physicians.Our vision is to achieve the highest standard of clinical quality and safety in a compassionate and patient/family-centered environment. Our mission is a commitment to excellence in healthcare, with patients and families at the heart of all that we do.**Some duties include but are not limited to:**Administer and supervise the provision of Social Work and Care Management Services to the hospital inpatient and selected outpatient populations. Services are provided according to the regulations of the NYS hospital code, the Joint Commission on Accreditation of Hospitals and other professional and regulating bodies. Responsible for supervising the multidisciplinary discharge planning and case management functions of the hospital. Functions as the primary liaison for the hospital to area skilled nursing and other older adult care facilities and community resources. Responsible for the supervision of the hospital Financial Case Management program, Spiritual Care Program, Addiction Specialists, and the HELP program. The director oversees Interpreter and related activities and the provision of Homecare Services from community agencies. The director oversees the facilitation of transportation resources related to discharge and transfer of patients. Represents Highland Hospital on system wide initiatives surrounding length of stay, readmission, Skilled Nursing Home Placement (eRecord Build), Alternate Level of Care (ALC) patients, and Age-Friendly Health System. The director is part of the Highland Leadership teams including Clinical Operations, ICARE Re-Launch, HCAHPS, Workplace Violence, and COPD Quality.**Salary Range:**$100,000.00 - $150,000.00 annually_The referenced pay range represents the minimum and maximum compensation for this job.
