Position Summary At Stony Brook Medicine, a
TH Senior Staff Assistant/Discharge Assistant is a valuable member of our team, who assist with transitional office operations. Individual works closely with Nurse Case Managers and Social Workers regarding payer delays, authorization and denial notification. Individual will also serve as a liaison between hospital departments to improve department function and initiatives. Candidate will support case managers and social workers with discharge planning efforts, coordinate trainings and meetings for the care management staff and maintain departmental resources for staff.
Duties of a TH Senior Staff/Discharge Assistant for the Discharge Hub in the Care Management Department may include the following but are not limited to: - Manage department discharge efforts, including home care referrals, DME arrangements, and skilled nursing transitions.
- Manage and assist with transitional office operations including but not limited to post-acute authorization and transportation coordination
- Collaborates with families, patients, and caregivers to facilitate seamless care transitions.
- Work closely with nurse case manager/social worker team regarding payer delays, authorization issues and denial notification.
- Serve as a liaison between hospital departments that will correlate with department function and initiatives
- Prioritize PT/OT consults daily based on discharge readiness and authorization requirements
- Engage with clinical teams to secure necessary signatures and documentation
- Partners with Senior Management to define project scope, establish goals, objectives and timelines, ensuring alignment across stakeholders.
- Maintain and coordinate departmental resources effectively.
- Main timely and up-to-date- project tracking.
- Manage complex projects with multiple stakeholders
- Maintain and Coordinate departmental resources
- Develop competency and maintain up to date knowledge of hospital software i.e. Power Chart, Siemens, Eclipsys & other programs as acquired
- Coordinate trainings and meetings
- Maintain understanding of referral resources and discharge planning programs.
- Uphold high standards of customer service and maintain high levels of patient and family satisfaction.
The selected candidate may be required to work weekend hours on a rotation basis at 1X per month.Qualifications Required Qualifications: - At least 2 years of experience working in an academic medical center/clinical setting in an administrative and/or supervisory capacity
- Bachelor's degree
- Demonstrated superior written and oral communication skills
- Computer proficient in Electronic Medical Record applications
- Demonstrate knowledge of discharge planning and length of stay
- Leadership skills, ability to run meetings, rounds, and educate staff
- High level of customer service skills
- Demonstrated patient first, patient advocacy and service excellence
- Demonstrate ability to project management and action planning, team work and team leader focus
- Ability to cover multiple patient populations
- Weekday and Weekend flexibility
Preferred Qualifications: - NYS LPN License/other advanced clinical education
- Bachelor's degree in health science or other related field
- Advanced Degree Certificate
- Hospital Operation and Throughput experience
- Leadership experience working with C-Suite Executives
- Experience in Care Management
- Experience in Managed Care/Insurance Industry/MD Office
- Working knowledge of medical terminology
- Knowledge of regulatory requirements i.e. CMS, EMTALA, Managed Care, etc.
- Bi-lingual
- Experience with post-acute care coordination, including acute rehab/skilled nursing and transportation authorizations

Anticipated Pay Range: The salary range (or hiring range) for this position is $68,975 - $85,313 / year. The above salary range represents SBUH's good faith and reasonable estimate of the range of possible compensation at the time of posting. The specific salary offer will be based on the candidate's validated years of comparable experience. Any efforts to inflate or misrepresent experience are grounds for disqualification from the application process or termination of employment if hired. Some positions offer annual supplemental pay such as:
- Location pay for UUP, CSEA & PEF full-time positions ($3400)
