Overview The Discharge Coordinator RN is responsible for the planning and coordination of discharges from the NICN. Works alongside the medical team and case management team to ensure safe and successful discharges from NICN to home. Responsible for all Family education surrounding the patient's home needs as well as coordination of outpatient care. The RN in this position will: Teach all family educations classes including, but not limited to CPR, Tracheostomy management, Gastrostomy Tube management, Ostomy management, etc. Works alongside Case management team to coordinate outpatient care Communicates with and Schedules all follow-up specialty appointments Works with department's educator to assist in Staff education regarding D/C needs Teaches Unit CPR classes Maintains all required certifications Attends daily huddle and weekly Coordiators meeting to plan for DC needs within the interdiciplinary team Participate on the education Committee and teach monthly NICN/NPCN education classes With some additional certifications preferred, which can be obtained once in the position: Obtain maintain CPR Instructor certification to teach Infant CPR Classes Obtain maintain NC Child Passenger Safety Technician Certification (car seat certification) through the State of NC Serve as a Car Seat resource for NPCN (and NICN when needed) Teach Car Seat classes for neonatal division Offer education (in-services/lunch learns, etc) for teammates regarding car seat use/updates, etc Certifications may be obtained after acceptance of the position but must be completed within 1 year Schedule: The schedule and hours for this position may vary, but will typically be on day shift 8a-3:00p M-F The position may require an evening and/or weekend shift when needed based on discharge or staffing needs. The D/C Coordinator RN is expected to work one 12hr holiday each season, in staffing adjust their schedule accordingly during a holiday week. NICN and NPCNs D/C Coordinator RNs are expected to coordinate their schedules and should not take PTO at the same time In the Neonatal Intensive Care Nursery , we provide critical, life-saving newborn care. We area level IV NICU with 53 licensed beds. We are the referral center for the region, caring for the most acutely ill and vulnerable patients. From the smallest premature newborns with complicated heart disease, we care for babies with every neonatal condition. We offer an in-depth onboarding process for those without previous NICU experience. Candidates must have at least 6 months of RN experience, this position is not for new graduates. Job Summary The RN Clinical Nurse I provides professional nursing care through skillful assessment, diagnosis, outcomes identification, planning, implementation, and evaluation in accordance with facility or department policies procedures, and/or protocols under the supervision of the nursing leader. Assists in maintaining a safe work environment and performs all related job responsibilities in a safe manner. Maintains clinical and professional competency as appropriate to the population of patients served. Essential Functions Collects data pertinent to the healthcare consumer's health or the situation. Analyzes the assessment data to determine actual or potential diagnoses, problems, and issues. Identifies expected outcomes for a plan individualized to the healthcare consumer or the situation. Physical Requirements Work requires walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling. Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies. Must speak English in good, understandable terms. Intact sense of sight, hearing, smell, and touch. Finger dexterity. Critical thinking and ability to concentrate. Must be able to respond quickly to changes in patient and/or unit conditions. Physical Abilities Testing required. Education, Experience and Certifications. Graduate from an accredited School of Nursing. BSN preferred. Current Basic Life Support required per policy guidelines. Current RN license or temporary license as a Registered Nurse Petitioner in the state in which you work and reside or; if declaring a National License Compact (NLC) state as your primary state of residency, meet the licensure requirements in your home state; or for Non-National License Compact states, current RN license or temporary license as a Registered Nurse Petitioner required in the state where the RN works.
