What you will do in this role:
* Coordinates discharge planning for a specified patient population in a single hospital.
* Follows patients throughout the continuum of care and ensures optimum utilization of resources.
* Provides ongoing support for discharge planning through comprehensive assessment of discharge needs to include barriers to compliance with recommended plan of care post discharge.
* Provides support and discharge planning for patients who are in need of ongoing medical/psychosocial care following discharge.
* In collaboration with other members of the healthcare team, assesses psychosocial needs of the patient and offers suggestions and facilitates the transition to follow-up care options.
Qualifications:
* High school diploma or equivalent.
* Bachelor's degree preferred.
* CPI/Handle with Care
* CPR
* One year experience in a psychiatric inpatient environment.
