Job Title: Medical Social Worker (LMSW/LCSW) Job Summary: The Medical Social Worker will provide advanced clinical evaluations and treatment to individuals requiring medical or mental health services within a hospital setting. This role involves independent decision-making in addressing complex patient needs, including psychosocial assessments, counseling, and discharge planning. The Medical Social Worker will act as a consultant to other clinicians and work collaboratively with the interdisciplinary team. Additionally, this position will offer leadership and training to entry-level social workers and social work students. Expertise in working with specialized populations such as oncology, renal, or pediatric patients is highly preferred. Key Responsibilities:
- Conduct psychosocial assessments of patients and families, presenting findings to the interdisciplinary team.
- Provide counseling to patients and families, focusing on the emotional and psychological impact of illness or injury.
- Coordinate discharge planning, making referrals to home health care, rehabilitation facilities, and community resources.
- Participate in interdisciplinary team meetings and case conferences.
- Provide in-service training on social work topics and offer guidance to entry-level social workers and students.
- Stay informed on the latest developments in medical social work and relevant disease processes.
- Represent the hospital at community meetings and foster collaboration between community services and hospital programs.
- Maintain accurate documentation and contribute to departmental statistics.
Qualifications:
- Experience: Minimum of 2 years of professional social work experience in a hospital, mental health, or human services setting.
- Education: Master's degree in Social Work (MSW) from an accredited School of Social Work.
- Licenses/Certifications: Valid LMSW or LCSW license issued by the New York State Education Department. Must maintain licensure throughout employment.
Required Skills:
- Strong understanding of medical social work principles, disease processes, and medical treatment modalities.
- Expertise in psychosocial counseling and crisis intervention.
- Experience working with complex cases in a hospital setting, especially with catastrophic illnesses and traumatic injuries.
- Ability to work effectively within a multidisciplinary team.
- Leadership skills in training and mentoring junior social work staff.
- Proficient in using computer applications such as spreadsheets, word processing, and database software.
Preferred Experience:
- Hospital-based social work experience preferred (nonprofit or outpatient backgrounds may not be as applicable).
- Experience working with diverse patient populations, including those with chronic illnesses or requiring specialized care.
