Role and Responsibilities The Diverse Care Project Director is responsible for overall management and coordination of HCR's project related to overcoming health care delivery disparities among African American senior citizens. This position will serve as the point of contact in the community to manage and nurture relationships with Black religious leaders who may refer individuals who need home care services. The Project Director will manage a small team of community liaisons who will help execute the project goals. The Project Director will be knowledgeable on home health care, maintain exceptional customer service, and work within the schedules of the Black churches, which may include evenings and weekends. Essential FunctionsGrant Management
- Oversee all aspects of the grant administration and management process including, but not limited to, planning, budgeting, tracking deliverables, submitting reports, and presenting accomplishments.
- Assure full compliance with grant guidelines, internal controls, and legal requirements
Grant Activities
- Implement the scope of work laid out on the grant project plan
- Develop and implement outreach to Black churches and delivering patient referrals for Black seniors, according to the grant's scope of work.
- Lead a small team of community liaisons to assist with outreach and administrative tasks.
- Responsible for appropriate messaging and positioning of home care services to each church group, including delivering informal and formal presentations with content-appropriate materials.
- Outreach functions may also include representation at conferences, health fairs, community events, as well as planning and participating in CME and non-CME educational opportunities.
- Other duties as assigned.

- Minimum of BS/BA required.
- Project Management certification preferred.

Qualifications and Requirements
- Must have 5 or more years of grant management experience.
- Must have experience in relevant outreach and/or community organizing experience, including 1 to 2 years of experience working with Black communities.
- Proven track record of successful outreach performance, relationship building and growing relationships.
- Excellent written, verbal, and presentation skills.
- Excels at providing customized customer service.
- Proven ability to drive results and use data to manage performance.
- Strong interpersonal skills and team building capability.
- Ability to manage multiple tasks/relationships at once.
- Highly self-motivated and works well independently but is also a strong team player.
- Ability to work nights and weekends as needed.
Work Environment The Diverse Care Initiative Project Director is a hybrid office/non-office setting and may be exposed to outdoor conditions. The working conditions are classified as light work:
- Light work - Exerting up to 20 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Light work involves sitting most of the time.
