Job Type Full-time Description Summary of Job Responsibilities: Act as an advocate for persons served and their families. Provide direct supervision and support for assigned Program Directors/Coordinators and Residential Supervisors. Monitor service quality and individuals' satisfaction, oversee internal case management systems, provide regular programmatic oversight in the areas of medication administration, financial management, clinical supports, healthcare coordination, asset management and billing. Provide on-call support and assistance. Maintain regular contact with all stakeholders, uphold and support the organization's mission, philosophy and values. Assist in creating person-centered, supportive learning environments that safeguard and enhance the lives of the people served. Essential Functions & Principal Duties & Responsibilities:
Promote and protect the rights and dignity of each person served and extend these rights and dignities to family members and guardians. Continually monitor support relationships for potential abusive or neglectful situations. Safeguard the confidentiality of each person served. Provide direction, leadership and supervision to all direct reports, programs & services. Oversee the coordination of CHS clinical, healthcare, case management systems and practices using a demonstrated working knowledge of Acumen, Shiftview, People Guru, HCSIS, Google and other company systems. Provide oversight to the implementation of the ISP process and other organizational processes. Advocate on behalf of each person served. Participate in transition planning and lead the implementation for both intakes into services and discharges from services. Monitor programs and services for quality, ensure established service standards are achieved. Participate in satisfaction surveys and other stakeholder satisfaction surveys and implement changes required to meet/exceed standards. Oversee, manage & coordinate the operations of all assigned programs including CHS asset protection (house structure, furnishings, appliances, vans, computer equipment, TVs), and risk management (health and safety of individuals and employees). Provide emergency on-call support and assistance to assigned programs and services. Monitor all fiscal and budgetary requirements and obligations. Participate in the development of program budgets. Manage outstanding payments due to CHS Inc. from families, guardians, individuals. Support the development of positive relationships, develop and implement of natural support strategies. Participate in community development and organizing efforts to ensure individuals regularly spend time outside their home. Act as the liaison for, and communicate on a regular basis in a supportive, professional manner with the individual served, family members and/or guardians, other service providers, management personnel, funding and referral sources, and other stakeholders of the organization as assigned. Oversee all data collection processes and ensure the accuracy, timeliness and integrity of information collected and reported. Generate and provide statistical and narrative reports on a monthly, quarterly, and yearly basis such as program evaluation measures, consumer assessments, family assessments, utilization/capacity, grievances, incidents, and provide written reports as required Oversee and ensure the provision of training and professional development for all employees in assigned programs and services. Ensure compliance with local, state, and federal laws and regulations as well as the policies and procedures of CHS, Inc. Recruit, screen and hire qualified employees in conjunction with individuals supported and their families as indicated. Establish performance standards and regularly evaluate employee performance. Uphold and comply with all safety programs and policies to maintain a safe work environment for the people we support, employees, family members and visitors. Participate in inter and intra departmental meetings; work in cooperation with others. Commit to, and participate in organizational initiatives and quality programs, to ensure improvements and develop effective processes. Act as a positive role model in dress, hygiene, demeanor and behavior for all individuals served. Maintains informal and formal timely communication with the supervisor and provide written communication when required. Perform other duties as directed by the supervisor related to the organization's mission, goals and operations. Internal/External Relationships:Internal: Frequently and on a continual basis with employees from all departments within the organization. External: Frequently and on a continual basis as described in responsibilities, including persons served, family members, DMR, service providers, other stakeholders, and community members, etc. Requirements Working Conditions: Varied and flexible to include office conditions (telephone, computer, etc.), external agencies/ businesses, at the location where the individual is receiving services (homes of the person served, etc.); in local community groups, etc. Physical & Mental Requirements: Ability to deal with a wide range of intellectual problems and practical problems. Ability to utilize excellent judgment in developing sound solutions, while ensuring the safety of the persons served and employees. Combination of sitting, standing, and walking. Must be able to lift 30 pounds. Ability to successfully perform the essential functions of this position with reasonable accommodations considered to enable individuals with disabilities to perform the essential functions. Education & Experience: Bachelor Degree in a related human service field with at least five (5) years of experience supporting people with developmental disabilities and/or mental retardation. Minimum of five (5) years of progressive administrative/management experience. Graduate Degree preferred. Excellent verbal and written communication skills, demonstrated supervisory skills, knowledge of community-based supports. Valid driver's license required.
