GENERAL DESCRIPTION
The Intake Specialist(s) will be responsible for monitoring incoming faxes and documentation for the organization. The documents received may include, but is not limited to, new patient referrals, clinical notes, prescriptions/ orders, nursing notes and insurance payor notification. The Intake Specialist will notify the sender of receipt of documentation, will be responsible for identifying the document type and will carry out necessary actions in accordance including notifying the internal team. New patient referrals will be entered in CPR+ and demographic information will be extracted and entered in the chart.
QUALIFICATION REQUIREMENTS
* Electronic health medical record experience
* Basic level skill in Microsoft Office (including Word, Excel, PowerPoint, etc.)
QUALIFICATIONS PREFERRED
* High school education or equivalent; life experience will be considered in lieu of education. College credits or degree a plus
* Health care experience; prefer homecare experience
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Intake Responsibilities:
* Continuously monitor incoming faxes in CPR+ fax queue and any other method of referring as applicable
* Review incoming documents and determine document type
* Notify sender of receipt of document
* Attach documents in CPR+ patient charts as necessary with accurate naming conventions
* Enter new patient profile into CPR+ when referral is received. Complete all demographic data entry fields accurately and in as much detail as possible.
* Complete a To Do in CPR+ to notify teams of new information/ document or when a new patient is added in the database
