JOB TITLE DATE Developmental Disabilities Full time/Part time/ Relief DIVISION POSITION STATUS Coordinator of Community Habilitation Non-Exempt TITLE OF IMMEDIATE SUPERVISOR FLSA STATUS Summary :Under the supervision of the Coordinator of Community Habilitation Care programs, the Community Habilitation Specialist is responsible for ensuring delivery of quality services to all individuals served. Hab. notes monthly according to standard.•Writes monthly summaries for each individuals served.•Inspect vehicles prior to use to ensure that they are in safe operating condition.•Develop individual activities according to Staff Action Plan.•Responds to all statements of deficiencies issued by the state.•Participates in pre-intake screening if it would be necessary•Participate in Life Plan meetings if it would be necessary and complete meeting minutes.•Provide CH services face to face to assigned individual.•Responsible to maintains annual trainings update•Able to work one day in the weekend•All other duties as assigned by the supervisor. Authority: This position exercises considerable initiative and independence in setting goals, activities and individual reviews. Under the general supervision of the coordinator, the specialist will maintain a professional and responsible work environment for the program.
