Job Description:

Implements activities suggested by the supervisor which reflect training objectives in each client's ISP (individual service plan). Engages residents in activities of choice, which reinforces ISP goals. Promptly reports any emergency/crisis situation to supervisor and initial agency Critical Incident Report within twelve (12) hours. Provides transportation and supervision for residents as dictated by individual programs. Ensures that all information regarding daily activities, problems and needs of residents are recorded properly in daily log. Ensures each resident's personal hygiene and grooming needs are met. Provides training and assistance in changing appropriate apparel on a daily basis to ensure comfort and well being for residents.

H. Assists residents and/or pack lunch preparation as needed.

I. Uses appropriate body mechanics to lift, move, and position or provides prescribed exercises and physical care to residents.

II. Responsibilities Relating to Health and Safety A. Inspects home at beginning of shift to ensure that home meets all safety requirements, checking for clutter and dangerous objects. B. Corrects deficiencies or submit written reports to supervisor outlining deficiencies by end of shift. C. Conducts evacuation drills, both fire and severe weather, as directed by the supervisor. Records results of each drill. D. Follow program plan for meeting all potential emergencies and disasters, such as medical, fire, severe weather and missing persons. Discusses with the supervisor specific role relating to each emergency. E. Notifies supervisor of all medical emergencies as outlined by policies and procedures. F. Checks to ensure that all flammable materials are properly stored and safeguarded. G. Checks to ensure that supplies are clearly labeled. H. Checks to ensure that food is protected from contamination and spoilage. Check the freezer and refrigerator to ensure that food is properly wrapped and/or covered. Corrects any problem and reports to supervisor for further corrective action. I. Assist clients with cleaning and sanitizing all appliances, utensils and work surfaces in accordance with the Personal Care Home Rules and Regulations guidelines. J. Checks cooking surfaces, exhaust hoods and oven (staff). Properly instruct clients on washing/cleaning and maintaining stove top in a safe manner. K. Disposes of garbage daily sanitizing containers and lids. L. Checks plastic ware, china, glassware or other similar items for chips or cracks. Places in a box to be picked up and removed from inventory in accordance with policy/procedure of New Connections Pathways Residential Services. M. Reports any problem with equipment used by residents to the supervisor. N. Jointly with client cleans and sanitizes bathrooms, kitchen, and home daily in accordance with Personal Care Home guidelines.

III. Housekeeping Responsibilities A. Checks home and instructs clients of all weekly household cleaning chores to include, but not limited to, inside windows, window sashes, blinds, mirrors, carpets, etc. B. Ensures that laundry is washed, properly put away daily, staff will assist/monitor with ironing. C. Other housekeeping responsibilities as assigned by the supervisor.

IV. Administrative Responsibilities A. Ensures compliance in the area of responsibility during assigned hours with local, state and federal rules and regulations applying to Personal Care Home and New Connections Pathways Residential Services. B. Complies with the organization policies and procedures governing the use of behavior management programs for controlling maladaptive or problem behavior. C. Complies with policies and procedures to ensure that confidentiality requirements are upheld. D. Supports and protects the fundamental human, civil, constitutional and statutory rights of clients and families as defined by agency. E. Demonstrates competence in handling medical emergencies in accordance with New Connection Pathways policies and procedures. F. Maintains accurate and complete records of all resident's personal spending monies in accordance with rules and regulations. G. Reports to work promptly and on time and does not leave work until the assigned shift ends or has approval from the supervisor. Observes policies/procedures governing requests for annual, sick or other leave time. H. Observes all other personnel policies and works closely with the supervisor to ensure compliance. I. Abides by code of ethics and serve as a good role model for clients.

V. Record Keeping Responsibilities A. Maintains records in the home to meet local, state and federal guidelines B. Makes client related entries in daily logs that are legible, dated, authenticated by signature and positions, in ink /typed and without use of symbols or abbreviations. C. Maintains tracking records in accordance with Individual Service Plan to comply with program requirements and expectations of the supervisor. D. Records contacts with the client's family or guardian.
