Job Type Full-time Description Position Title: Health Services Specialist/Residential Services Department: Residential Reports to: Health Care Coordinator FLSA Status: Exempt Telework Eligible: No POSITION SUMMARY Responsible for performing a variety of duties to provide quality nursing care to residents and to coordinate total nursing care for residents; implementing specific procedures and programs. Supervision of staff assigned as medical support. DUTIES AND RESPONSIBILITIES
Provide a supportive, caring, safe environment by attending to individual needs, respecting differences, and supporting growth and independence. Hiring, training, and supervision of health care personnel in residential programs. Involvement in referral process (intake, assessment, placement, transfer) and supervision of referrals as requested. Preparation of required or requested reports. Attend meetings as required and act as a member of the Interdisciplinary Team. Evaluation and monitoring of medical needs of all individuals and to ensure that appropriate health services are provided in collaboration with program staff. Coordination of scheduling and attendance at individuals' medical appointments as needed. Coordination of medication records and administration of prescribed medication. Complete assessments of residents and analyze information to develop appropriate interventions to prevent decline; record signs of change in condition; notify appropriate staff per facility protocol to evaluate weight loss, decline in ambulation, development of skin breakdown, etc.; observe conditions which indicate possible need for restorative nursing programs and then notify appropriate staff per facility protocol of the need for evaluation; inform the physician of changes in assessment when appropriate. Administer medications and treatments to residents; place orders for medication from the pharmacy. Assess residents upon admission or re-admission to develop interim care plan and further assess in depth to contribute to the complete care plan. Attend and participate at ISP meetings as scheduled. Respond to emergencies; assess the program participant's condition; decide on the type of medical attention; call appropriate ambulance service if needed; notify family when program participant resident is transferred or admitted to the hospital; notify the physician when a change in condition occurs. Maintain program participant clinical files; keep charts updated; document appropriately. Make rounds to see each program participant routinely and as needed. Maintain accountability for controlled substances; inventory medications at the beginning and end of the shift; notify supervisor of discrepancies. Correlate accurate and complete documentation and its impact on reimbursement and federal regulations. Communicate status of program participants to incoming personnel according to established procedures; communicate with physicians, transcribe, clarify, and implement physicians' orders. Supervise medical support personnel. Provide on-the-job training, evaluate performance, and provide direction. Regular attendance at work. Other duties as assigned. Requirements MINIMUM QUALIFICATIONS
Graduate of an approved school of nursing; registered nurse or LPN with a current license to practice in the Commonwealth of Pennsylvania. Experience with or knowledge of acceptable assessment instruments used in evaluation of human growth and development Experience working with individuals with intellectual and physical disabilities preferred. Valid PA driver's license. Assist with maintaining program participant clinical files; keep charts updated; document appropriately. 13. Assist with HRST rating as requested and complete nurse reviews in a timely manner. 14. Review controlled substances count sheets; inventory and review medication expiration dates and availability as well as confirming pharmacy label matches CaraSolva regularly; notify program supervisor of discrepancies. 15. Communicate status of program participants according to established procedures; communicate with physicians, clarify, and implement physicians' orders. 16. Provide on-the-job training, evaluate performance, and provide direction. 17. Regular attendance at work. 18. Other duties as assigned. COMPETENCIES

PHYSICAL REQUIREMENTS AND WORK ENVIRONMENT

May be required to reach above the shoulder, below the waist or lift documents or materials throughout the workday. Proper lifting techniques are required. The Health Services Specialist is required to lift and/or move up to 35 pounds. May be required to walk up and down stairs periodically throughout the day. Finger dexterity and close vision ability are required.
