This position is intended to maintain and enhance a therapeutic environment that promotes the independence and quality of life for our residents. The Vitality Coordinator will assist in analyzing and improving the internal transition process. This position reports to the Director of Community Life.Essential Job Functions:Conduct Vitality Assessment of potential new residents. Schedule and perform Vitality Assessments (wellness assessments) on Independent Living residents including counseling and educating residents regarding their Vitality Score and referring to appropriate services if needed.Facilitate the community-wide transitions process through coordination of weekly meetings with the Transitions Team.Remain current regarding the healthcare and cognitive capabilities of the Independent Living residents. Maintain current Independent Living resident medical information in AOD as provided by residents according to the policy and procedure of the department and applicable regulations.Coordinate support services, i.e. home care, physical therapy, home health, etc. for Independent Living residents.Coordinate care conference meetings between Independent Living residents, family members/responsible parties, and appropriate staff.Provide support, comfort, and reassurance to resident and family/responsible parties regarding the care giving needs of the resident. Encourage family involvement whenever appropriate.Counsel residents regarding maintaining a safe living environment in their homes and follow through to ensure necessary changes are made by resident.Communicate with healthcare providers (physicians, therapists, ancillary providers, case managers) families, and responsible parties of Independent Living residents.Conduct hospital and rehab visits as needed to coordinate discharge plans.Adhere to the policy and procedure of the Resident Services Department and the organization, including but not limited to Infection Control Plan, Exposure Control Plan, and Emergency Management Plan.Respond to all fire alarms and assist in the evacuation of residents when needed.Respond and assist when residents are reported missing.Facilitate educational sessions for residents on general healthcare topics.May be required to evacuate with Independent Living Residents in the event of a weather-related emergency.Utilize accepted communication tools and channels effectively, i.e., two-way radio, cell and phone systems, computers, Quikkast emergency messaging system and communication with other department managers and staff.Maintain confidentiality of resident/family and facility information.Provide quality customer service in all interactions with residents, guests and staff.Perform other duties as required.You Have:One to three years' experience in a geriatric setting, i.e., skilled nursing facility, assisted living facility, medical or non-medical home care, or geriatric acute care settingLPN/RN or Social Worker preferredStrong organizational skillsCPR certificationWe Have:Career advancement opportunitiesCompetitive salariesComprehensive benefits package, including medical, dental, life and disability insuranceEducational assistance programsPaid Time Off, company paid holidays, 403(B) plan with matching contributions, and moreRead Important Disclaimers and Authorizations:Please click here to open important information you need to know before applying to a position at SantaFe Senior Living, Inc. or one of its three affiliate communities; The Village at Gainesville; The Terraces at Bonita Springs; East Ridge at Cutler Bay.
