This position is full time, Monday - Friday. Employees are required to have flexibility to work any of our shift schedules during our normal business hours of 8:30 AM - 5:00 PM. We offer 2 weeks of paid training. The hours during training will be 8:30 AM - 5:00 PM from Monday - Friday. Our office is located at 3 Farm Glen, Farmington, CT. If you are located in Farmington, CT, you will have the flexibility to work remotely* as you take on some tough challenges.Primary Responsibilities: Non - patient facing role with primary function to interface with ED providers to facilitate ED discharges with a safe plan and follow - up Monitor ED / Inpatient admissions to facilitate a safe outpatient follow - up Review inpatient and ED reports Schedule outpatient appts for members Make outbound calls to members Collaboration with providers to avoid high ER utilization and inpatient admissions Ensure safe transition of care Chart review of multiple electronic health records Documentation and tracking of outreach activity Preparing case summary to present for weekly clinical discussions You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Required Qualifications: Certified Medical Assistant, Certified Nursing Assistance, OR Personal Care Attendant Must be 18 years of age OR older 2+ years of experience in a physician's clinic OR hospital Additional experience with working in a medical care setting as a receptionist OR medical assistant Basic knowledge of chronic medical conditions Medical terminology knowledge base Possess and demonstrate excellent organizational skills, customer service skills, including verbal and written communication Ability to work independently, with some supervision and direction from manager Ability to maintain and demonstrate a high degree of professionalism to include both personal conduct and appearance at all times Intermediate computer skills, which includes navigating electronic health records and use of various Microsoft Office software Ability to work any shift between the hours of 8:30 AM - 5:00 PM from Monday - Friday Preferred Qualifications: 2+ years of administrative support experience Familiarity with clinical triage algorithms Ability to work independently and collaboratively in a remote setting Telecommuting Requirements: Reside within commutable distance to the office at 3 Farm Glen, Farmington, CT Ability to keep all company sensitive documents secure (if applicable) Required to have a dedicated work area established that is separated from other living areas and provides information privacy Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service *All employees working remotely will be required to adhere to UnitedHealth Group's Telecommuter PolicyConnecticut Residents Only: The hourly range for this role is $19.47 to $38.08 per hour.
