Job DetailsJob LocationNew York, NYPosition TypeFull-TimeEducation LevelBA/BSSalary Range$45,000.00 - $45,000.00 Salary/yearJob ShiftMonday - Friday 9 AM - 5 PMDescriptionUnder the direction of the Supervisor and/or Director, the Engagement and Enrollment Specialist provides outreach and enrolls eligible individuals in Care Coordination services and assists with member retention needs. Services are provided for Medicaid and Non-Medicaid recipients living with co-occurring medical, mental health, and/or substance abuse needs.The Engagement and Enrollment Specialist conducts telephonic, written, and face-to-face outreach and retention services to coordinate care for Health Home Care Coordination eligible members. The Engagement and Enrollment Specialist works closely with the Care Coordination team to identify, locate, engage, and enroll members eligible for Health Home services. The Engagement and Enrollment Specialist assists the team by providing information about community resources that can support member wellness goals.Essential Position Functions:
  • Provide telephonic, written, and face-to-face engagement and enrollment services for each Health Home eligible individual to facilitate Health Home enrollment and retention.
  • Utilize internet databases, health record systems, and other resources to explore individual eligibility and contact information.
  • Utilize Electronic Health/Medical Record system(s) of assigned Health Home(s) and maintain ongoing clinical documentation and records upholding all HIPAA regulations and confidentiality protocols to record each interaction with every individual served.
  • Assist Care Coordinators in securing and identifying needed referrals to community and network medical, behavioral health, and social assistance providers through telephonic, written, and/or face-to-face outreach, research, and maintenance of the program resource directory.
  • Provide follow-up services via telephonic or face-to-face engagement with members and service planning partners as needed to coordinate reminder calls, medication, and wellness checks upon request from the Care Coordination team.
  • Participate in conjoint meetings or member transports with other Care Coordination staff.
  • Provide oral and/or written status updates regarding member alerts, progress, regress, and needs to responsible Care Coordinator(s) and Supervisor, and any service providers, legal mandate, or other care plan affiliates as needed to assist the program and members.
  • Complete all UAS assessments for HARP enrolled members upon enrollment
  • Attend meetings and case conferences in the community.
  • This position requires the candidate to be on the field about 75% of the time and take mass transit to travel from one site location to another.
  • Any other duties as assigned.
QualificationsRequired Knowledge, Skills, and Abilities:
  • B.A./B.S. level professional with at least two years of experience working with the Mentally ill/Substance Abuse population. Minimum of 2 years in a mental health setting.
  • Experience with electronic record systems is strongly desired.
  • Willingness to travel by public transportation and the ability to manage time and meet program / regulatory requirements are essential.
  • Must have experience in outreach, client engagement, assessment, direct service and treatment, and documentation.
