As an Engagement & Outreach Counselor, you will be responsible for providing a broad array of crisis supports for patients of Berks Counseling Center. - Provide outreach to all persons served discharged from inpatient facilities on the day of discharge.
- Meet with all persons served following discharge from an inpatient facility within seven (7) days.
- Provide counseling and other clinical supports as needed to fill gaps in care related to PTO.
- Provide support to persons served who need inpatient care.
- Coordinate referral and admission to inpatient care when appropriate and within BCC policy.
- Provide care to persons served in an inpatient setting to ensure ongoing support and positive transition back to BCC's care.
- Provide screening, assessment, and evaluation of patients experiencing a crisis, either on site, on the phone, or in the community.
- Determine severity of each crisis call or encounter.
- Record crisis phone calls in a Critical Incident Log and complete Critical Incident Report.
- Refer patients to community-based emergency response systems or other appropriate services.
- Work collaboratively with SAM, TASC, hospital emergency departments, inpatient treatment facilities, and law enforcement.
Requirements To qualify for the position, you will need...
- A bachelor's or master's degree in a related social service field
- At least one (1) year direct counseling experience (or relevant internship).
- A Pennsylvania Certification Board (PCB) certification (CRS, CAC, or CADS) or the ability to get it within 1 year of employment.
- Valid PA Driver's License and the ability to drive, travel and provide community-oriented services required.
- Ability to work 40 hours per week within agency hours of operation, which are 8am-8pm Mon-Thurs, 8am-4pm Friday, with Saturday hours, when applicable.

- SEEING: Must be able to see well enough to read reports and use computer. Must be able to see well enough to drive an agency vehicle.
- COMMUNICATING: Must be able to communicate to clients, vendors and coworkers.
- MOBILITY: Must have mobility to move about the agency. Must have mobility to move about the community.
- LIFTING/PULLING/PUSHING: Must be able to lift up to 5 pounds
- OTHER: Must be able to write, type and use technology. Must be able to operate a motor vehicle.
Preferred candidates will ...
- Master's degree in a related social service field with at least three (3) months direct counseling experience (or relevant internship).
- LSW, LCSW, or LPC applicants with counseling experience in drug, alcohol and/or mental health.
- Be Bilingual: English/Spanish.
- Have PA Certification Board: Certified Alcohol Counselor.
- Have Knowledge of or experience with trauma-informed approaches preferred.
- Know the assessment and treatment of behavioral health disorder.
- Know community services available to low-income, high-risk populations.
- Have experience in documenting clinical needs clearly, concisely, and in a timely manner.
