Enrollment Specialist for its location in Broad Street PA. The primary purpose of the Enrollment Specialist position is to serve as the contact for newly qualified referrals, providing overview of PACE model and obtaining initial information relating to clinical and financial eligibility. The Enrollment Specialist is required to promptly follow-up on all qualified referrals assigned to them in the PO's Customer Relationship Management Platform as well as accurately document activity in the Electronic Health Record. This position works with the enrollment team to manage qualified referrals through the entire enrollment process and promotes the PACE Program to referrals, family/primary contacts, and community agencies the potential referral may be associated with. This position requires localized travel to referral sources such as hospitals and rehabilitation centers as well as home visits to potential clients. Occasional evening or weekend hours may be required. Work schedule is typically Monday through Friday, 8 am until 5 pm. Programs of All-Inclusive Care for the Elderly (PACE) coordinate and provide all needed preventive, primary, acute and long-term care services so that older individuals can continue living in the community. PACE is an innovative model that enables individuals who are 55 years old or older and certified by their state to need nursing home care to live as independently as possible. Through PACE, today's fragmented health care financing and delivery system comes together to serve the unique needs of each individual in a way that makes sense to the frail elderly, their informal caregivers, health care providers and policy makers. Many PACE participants get most of their care from staff employed by the PACE organization in the PACE center. PACE centers meet State and Federal safety requirements and include adult day programs, medical clinics, activities, and occupational and physical therapy facilities.

Essential Functions include but are not limited to: Serve as contact for prospective enrollees and/or their family/primary parties to provide information about PACE. Obtains demographic, medical, insurance, financial, and referral information to determine eligibility and interest. Refers individuals to other appropriate resources when necessary. Establish and maintains positive relationships with prospective enrollees, which may include visits to homes and/or other settings. Responsible for reviewing enrollment documentation and ensuring potential participants understand program services. Acts as liaison between prospects and staff as necessary, for communication, scheduling, care plan development, and enrollment. Responsible for inputting required documentation into the PO Customer Relationship Management Platform (CRM) and Electronic Health Record (EHR) in a timely manner. This includes and is not limited to new referral opportunities, contact information for referral family or primary contact, referring company, referring contact and all related activities associated with each opportunity, referral source, family/primary contact and referring contact. Coordinate and continually monitors each component of enrollment process to ensure all deadlines are met, initiating changes as necessary. Schedule's enrollment assessment process and maintains documentation in various computer programs for tracking and reporting purposes. Assist with Medicaid financial application process. Reviews Medicaid eligibility and provides administrative support, monitors approval time frame, and other tasks to facilitate smooth application and certification. Communicate regularly with all PACE Organization (PO) members of the Interdisciplinary Team to share key information, coordinate tasks, and provide support for the enrollment process as needed. Establishes and maintains positive working relationships with referral sources to regularly discuss possible program candidates and to ensure a clear understanding of PO as a vital community resource. Maintains similar relationships with Medicaid and other industry contacts. Assist with sales activities as needed, including representing PO in community presentation, special events, health fairs, trade shows, etc.
Requirements: Associates Degree with a minimum of two (2) years of sales or marketing experience is preferred. A combination of education and experience may be acceptable. Valid driver's license Must be action-oriented, have business acumen, manage conflict well, be customer focused, have high decision quality, flexibility to adapt to ongoing change and have organizational agility. Ability to work with minimal supervision and exercise independent judgment. Demonstrates superior written and verbal communication and presentation skills appropriate for audience comprehension. Well-developed communication skills, both written and oral, that may be used either in an on-site or virtual environment is required. Able to communicate effectively with individuals and groups representing diverse perspectives. Comprehensive to expert proficiency with Microsoft product suite (MS Word, Excel, Power Point, etc.); basic knowledge and experience with electronic mail and calendaring system necessary. Ability to type with speed and accuracy. Ability to use other software as required to perform the essential functions of the job. Possesses a high degree of personal accountability, responsibility, and independent decision-making abilities with the skills to interpret programs, goals, objectives, policies, and procedures of the organization in line with mission, vision, and philosophy of TH PACE. Excellent organizational skills. Ability to perform multiple duties and functions related to daily operations and maintain excellent customer service skills. Ability to perform frequent detailed tasks and provide immediate service with frequent interruptions. Ability to change and be flexible with work priorities. Strong problem-solving skills. Ability to research, analyze and assimilate information from various on-site or virtual sources based on technical and experience-based knowledge. Must exhibit critical thinking skills and possess the ability to prioritize workload. Position requires periodic travel within the PACE Program's geographic region (up to 50%).
