SUMMARY: Under the supervision of the Enrollment Program Coordinator, the Enrollment Worker (EW) assists APHCV's patients and clients to enroll into health insurance and subsidy programs defined by the Enrollment Program Coordinator. The main goal of EW is to assist patients to enroll and maintain enrollment into health insurance and subsidy programs and to anchor patients to APHCV if they are interested. APHCV recognizes outreach as an optional strategy to achieve this goal. All duties and responsibilities shall be carried out according to APHCV's policies and procedures. Although Enrollment Services are housed in APHCV three Health Centers, if directed by their supervisors, EWs are responsible for carrying out their duties at other APHCV service locations. Regardless of where they are assigned to work, EWs are expected to adhere to APHCV's quality, standards, and performance and implementation of APHCV's policies and procedures. APHCV expects all employees to participate in APHCV's emergency response operations per our emergency policies and procedures. APHCV requires all staff to comply with Standards of Conduct and Compliance Program related policies and procedures. Such compliance is part of this position's performance evaluation. APHCV is a tobacco-free organization. DUTIES AND RESPONSIBILITIES:
Enrollment Services: Screen APHCV's patients and community members for eligibility in various health programs and CalFresh benefit and provide appropriate enrollment or referral or troubleshooting assistance. Function as on-site enrollment specialist for APHCV patients including assisting patients in completing applications and preparing for the interview with the DPSS eligibility worker and assisting them stay enrolled. Assist community members how to choose their Primary Care Provider (PCP), educate how to navigate health care system and to make medical appointments. Assist APHCV patients with PCP and Health Plan assignment if they are interested in becoming APHCV patients or continuing to receive services at APHCV. Assist seniors and disabled persons with Medicare and Medicare Part D enrollment, coverage, and supplemental programs as well as promote APHCV's clinical services to older adults and to Medicare eligible individuals. Provide follow-up, case management and troubleshooting of client's and APHCV's patient's cases (including enrollment cases as well as managed care patients and others as assigned by Enrollment Program Coordinator and/or the Unit Manager). Ensure proper documentation of all charts and follow-ups being made on monthly basis. Enter enrollment and its related information into designated programs such as Covered CA, One- E-APP and/or Your Benefit Now program. Maintain program participate files including enrollment documents, records of services accessed, and verification. Input participant enrollment information into CHOI or other database program on weekly basis. Provide monthly report and telephone log to Enrollment Program Coordinator by 10th of every month including a narrative or client/patient' story on services provided and progress made; a quantitative report on enrollment numbers reach per month, and all other program activities assigned to measure productivity and attach phone log and outreach sign-in sheet if any. Patient and Client Retention Services (Utilization of Benefits) Retain/Assist those who are assigned to APHCV Send welcome letter to newly assigned patients Contact them to make Initial Health Assessment (IHA) appointment Contact pending and redetermination cases to see patient needs help. Contact dis-enrolled patients to see patient needs help or understand why they dis-enrolled. Track return envelops and communicates to front desk, and follow up per procedure. All other activities assigned to assist retain patients and clients with APHCV. Record and/or log in all activities and completions of the assigned tasks including but not limited the above tasks. Outreach: Outreach is conducted when it is evident that it helps achieve the EW's main function to enroll patients and community members. Outreach plan shall be approved by the Enrollment Program Coordinator. Assume a liaison role between APHCV and appropriate community including businesses, temples, cultural centers, service organizations, schools, and other community institutions. Plan and implement culturally appropriate outreach activities to enhance their success in enrolling patients and community members or help them stay enrolled. Provide outreach productivity report and sign in sheet to Enrollment Program Coordinator and input outreach number into CHOI or other database program on weekly basis. Translation/Interpretation for the Enrollment Services activities. Conducts interpretation service for the Unit. If interpretation support is requested by other department, it shall be first approved by Enrollment Program Coordinator, and only when the protected scheduled time is set up to function as interpreter. Translate health education for the Unit. This may include flyers, information about the program activities, articles for newspaper, and other program related activities in appropriate language as assigned. Participate in the process to evaluate the accuracy of translation per APHCV procedures. Type set or coordinates with other bilingual staff to type set translated materials. Other: Submit any other reports and forms to the Enrollment Program Coordinator when requested. Provide the Enrollment Services office reception support, if needed. Answers incoming calls, retrieves answering machine messages and returns calls. Provide other support and assume other duties as assigned by Enrollment Program Coordinator or Unit Manager. PERFORMANCE EXPECTATIONS:
Achieve and maintain successful enrollment, retention of eligibility, and retention of patients, and meet productivity targets Abide by all Enrollment Services Policies and Procedures, requirements by funder's contracts or grants, and all other relevant Policies and Procedures of APHCV including but not limited to HIPAA Policies and Procedures, Confidentiality Policies and Procedures, etc. QualificationsQUALIFICATIONS:
- Be Covered CA Certified Enrollment Counselor
- Bachelor's degree in community health, social sciences, or related field, or minimum of 2 years of work experience in a related field may be substituted in combination with education.
- Ability to speak, read, and writes fluently in both English and a respective target language.
- Ability to work with people of diverse cultural, educational, socio-economic, and linguistic backgrounds
- Ability to organize information, filing, understanding of various program expectations and requirements.
- Ability to enter data into computer program at expected time allocation.
- Ability to use basic computer programs
- Strong communication and organizational skills
- Ability to work efficiently, accurately, and with minimum supervision.
- Ability to teach, instructs, or provides presentations to big groups.
- Interest and commitment in improving community health
- Skilled in time management and able to prioritize
- Good customer service skills
- Proficiency in Microsoft Office applications
HR Procedural requirements:
- Legal authorization to work in the United States
- A valid California Driver's license with clean records and access to insured automobile
- Completion of APHCV Health Assessment Form
- Completion of DOJ background check
PREFERRED SKILLS:
- Completion of industry standard 40-hour medical or health care interpreter training.
- Outreach and/or public health and/or community-relevant experience
- Knowledge and familiarity with the U.S health care system and API health issues
- Knowledge of health promotion and education
