As our agency grows in Erie County, OhioGuidestone is partnering with the Erie Health Department to provide additional services. We are seeking a Licensed Social Worker or Licensed Professional Counselor to fill the need for social work services in Erie County. Preference for LISW/LPCC or higher. This opportunity will mostly be office based with some community based activities. Hours will be M-F between 8:00 am-5:00 pm except Tuesdays where the Therapist would need availability until 7pm (can alter rest of week hours to make up for this). Office Based, No Productivity Standard, Competitive Salary, No Weekends Essential Functions: Conducts psycho-social evaluations and provides therapeutic interventions including crisis counseling to individuals, groups, and families. Evaluates the individual and family systems as well as assess client's functioning within work, family, and routines of daily living and identifies areas needing continued support, resources, and treatment in order to assist clients. Maintains client records to reflect course of treatment and provides required documentation. Provides follow-up and aftercare planning services as needed. Expeditiously discharges inactive clients. Provides emergency services during work hours and after hours as assigned. Prepares clinical assessments, treatment, case management, consultation, referral, and education according to the individual's professional discipline. Services shall be provided within the scope of practice of the individual. Collaborates with other staff members to perform clinical assessments and develops treatment plans. Consults about medications and other treatments to patients with Medical Director and/or physicians. Evaluates the effectiveness of counseling programs and clients' progress in resolving identified problems and moving towards defined objectives. Modifies treatment activities and approaches as needed in order to comply with changes in client's status. Participates in the analysis of data to identify trends, health problems, environmental health hazards, and social and economic conditions that adversely affect the public, behavioral and mental health issues, concerns and interventions. Develops and implements treatment plans based on clinic experience and knowledge. Coordinates public health, mental health, and behavioral health services with public health, mental health, and behavioral health professionals and investigators from other agencies and jurisdictions, including but not limited to, the Ohio Department of Health and Centers for Disease Control and Prevention. Collaborates in the development of evidence-based public health, mental health, and behavioral health programs in collaboration with universities, other nursing agencies, businesses, trade associations, other staff, and the public. Collaborates in the development, implementation, tracing, and evaluation of long and short-range policies, procedures, plans, and programs for the public health in Erie County. Collaborates in and contributes to individual, team, and Agency performance management, quality improvement, and evaluation activities. Adheres to ethical principles in the collection, maintenance, use, and dissemination of data and information. Maintains, updates, and uses computerized data management systems and utilizes informatics to improve program operations. Encourages clients to express their feelings and discuss what is happening in their lives, and help them to develop insight into themselves and their relationships. Utilizes appropriate methods of interacting effectively and professionally with personnel of all ages and from diverse cultural, socioeconomic, educational, racial, and ethnic backgrounds, sexual orientations, lifestyles and physical abilities. Provides leadership for the growth and expansion of mental health and behavioral health services offered by the Erie County Health Department and Erie County Community Health Center. Advocates on the behalf of vulnerable individuals and populations; participates in assessing and evaluating health care services to ensure that people are informed of available programs and services and are assisted in the utilization of those services. Participates in Agency and community emergency response training and drills in support of disaster preparedness consistent with job classification. Responsible for using quality improvement (QI) and performance management (PM) processes and/or techniques to improve the effectiveness of the public health programs. Assumes responsibility for own professional growth and development by pursuing education, participating in professional committees and work groups, and contributing to a work environment where continual improvements are pursued. Greets and treats visitors and stakeholders in a friendly, competent, professional and informed manner. Responds to questions with timely, accurate, and complete information. Answers and redirects incoming calls as appropriate. Follows all regulations and requirements pertaining to public health, mental health, and behavioral health, federally qualified health center (FQHC), and Agency policies towards the workplace. Provides CPST and minor case management functions if needed. Assumes a strong advocacy role for clients and makes recommendations for clinical and support services needed upon discharge. Actively participates in all treatment conferences and meetings for the clients. Attends conferences and workshops to maintain professional competence. Provides in-service training as requested. Maintains regular and reliable attendance. May be involved in the planning, organization, and implementation of special events or department-specific projects or seasonal program changes. Exhibits positivity, flexibility, and a willingness to take on new responsibilities as requested or required. Demonstrates positive leadership, promotes a team-based work environment and presents the agency in the most positive light with all internal and external contacts. Performance/Physical Requirements Work environment is primarily the agency office, but may include community-based activities. English reading and writing skills required. Must be available for crisis management by phone. Computer literacy skills, including Windows applications, Microsoft Office Suite, and basic database usage, as well as basic typing. Knowledge of clinical diagnose for mental and behavioral health population. Ability to problem solve and de-escalate crisis situations. Knowledge of the core functions and essential services of public health. Ability to observe patient behavior and report any changes in mental, behavioral, or physical health to medical staff. Knowledge of current trends in mental hygiene, mental health, public health, and social services, including State and Federal programs in these areas. Knowledge of general office principles and practices. Knowledge of action tracking on specific work assignments or other items related to work position. Requires a self-starter with the ability to work both independently and as a team member in a professional environment. Ability to use good judgement and persistence in overcoming challenges, addressing conflicts, and solving problems. Qualifications Minimum educational level is a Master's degree in Social Work, Counseling, or Education with a valid Ohio LISW or LPCC required. Experience with individual, family, and group therapy practice. Experience working with underserved populations or mental health population. Bilingual skills are a plus. Must have a valid Ohio Driver's License with a safe driving record and valid insurance. Ability to take and pass a physical exam and drug screening.
