We have an exciting opportunity for an External Case Manager to join the Heritage Manor Admissions department. The External Case Manager, will interface with local community hospitals, foster physician relationships, assist in the coordination and facilitation of admissions through clinical review of potential clients for Admission. Will work closely with Admission team to develop and promote a seamless referral to admission process while building a strong referral base. Demonstrates an understanding of, and embraces, the mission statement of Christian Health.

Competencies:

  • Refers sponsor to Admissions Director for completion of tours, admission paperwork, and financial information.
  • Identifies and assists in coordinating the acquisition of any needed special equipment, supplies, or consultations.
  • Documents appropriate information as indicated and distributes to MDS Coordinator, Supervisors manual, unit, Rehab, Social Services, COPS Nurse, and Food Services. The documentation will be timely, accurate, and concise.
  • Communicates with Admissions Director/DON/Administrator regarding census driven needs, including room changes, inter campus transfers, readmission's, etc.
  • Assesses appropriateness of care at Christian Health.
  • Conducts Pre-admission/re-admission assessment visits, providing the information to assist in the completion of the MDS and initial nursing assessment in accordance with regulatory guidelines.
  • Demonstrates awareness of legal issues in all aspects of resident care and facility function and strives to manage situations in a manner that reduces risk.
  • Acts as liaison with referral sources, i.e., hospitals, skilled nursing facilities, adult homes, assisted case managers and community outreach programs (makes house calls when necessary).
  • Assesses and screen residents for utilization issues and confers with IDC Team regarding specific cases.
  • Coordinates and facilitates the flow of patients into the skilled nursing facility.
  • Markets the facility programs and services to health-related audiences and consumers.
  • Tracks hospitalized patients across the long term care program in order to facilitate re-admission to the facility (when appropriate).
  • Participates in activities related to clinical outcome data collection, as well as performance improvement.
  • Identifies resident and interdisciplinary team educational needs and confers with Clinical Education department to provide educational resources to the interdisciplinary team (i.e., articles, handouts, posters, one-on-one teaching and/or demonstration) as well as coordinate resident related in-services.

Schedule: Tuesday Friday, 9am-5pm and Saturdays 8am-4pm.

Education: Bachelors degree in related field required.

QUALIFICATIONS/REQUIREMENTS:

Demonstrates compliance with the CH ASPIRE Standards of Performance.

Qualifications:

  • Minimum of 2 years related experience, preferably in case management.
  • Thorough knowledge of Medicare PPS/MDS in long term care setting preferred, marketing and program development.
  • Excellent interpersonal skills.
  • Maintains a liaison with the residents, their families, support departments, etc. to adequately plan for the residents' needs.
  • Computer knowledge a must.
  • Valid driver's license required.
  • Gerontology preferred.
