Facilitated Telehealth Coordinator - Tampa, FLA TEAM OF DIFFERENCE MAKERS:Author Health provides outpatient comprehensive behavioral healthcare and treatment which will improve health and general well-being through care coordination, connection to community-based resources, and technology facilitation for clinical visits. We are seeking a highly organized and motivated individual to join our team as a Facilitated Telehealth Coordinator in the HealthCare industry. As a Facilitated Telehealth Coordinator, you will serve as the in-person liaison between the patient and our virtual care team. This position offers an exciting opportunity to contribute to the advancement of healthcare technology and improve access to care for our patientsWHAT IS YOUR SUPERPOWER?Travel to patient homes or primary care offices to assist patient and/or caregiver with connecting to the tele-health visit - typically up to four visits/dayFacilitate communication between providers and their patients including assisting patients use of technology to conduct virtual visits with other providers on the teamAssess environment regarding safety concerns or potential hazardsCommunicate with the patient care team regarding any patient questions and needs such as medications, Community Based Organizations.Coordinate telehealth services and ensure seamless delivery of care to patients in various settings: home, clinic, living communities (ALFs, etc.)Provide education and support to patients and caregivers on how to use telehealth technology effectivelyMaintain accurate and up-to-date records of telehealth appointments and patient dataCollaborate with IT and technical support teams to resolve any telehealth system issuesConduct training sessions to educate healthcare staff on telehealth best practices and workflowsCollaborate with cross-functional teams to identify technology needs and opportunities for improvementTroubleshoot and resolve technology issues in a timely manner to minimize downtime.WHAT WE ARE SEEKING:Comfortable working with the senior population and those with behavioral health conditionsProven experience working in a healthcare setting or similar roleKnowledge of telehealth technology and softwareExcellent organizational and time management skillsStrong attention to detail and ability to multitaskExcellent communication and interpersonal skillsExcellent problem-solving and communication skills.Ability to work independently and collaborate effectively with team membersProactive and adaptable to change in a fast-paced environmentPossession of driver's license and carPreferred Bilingual (Spanish-English)Willingness to travel up to 100 miles round trip per dayPREFERRED:At least 1 year of experience directly related to the duties of the positionPassion for, and experience working with, patients with mental health conditionsPassion for, and experience working with, seniorsThis role does require walking to and from patients' homes in all weather conditions. This role demands a high level of flexibility, independence, and reliable transportation. In addition to in-person work, employees will occasionally work from home, utilizing virtual tools and software to manage tasks, communicate with team members, and complete administrative duties.
