Position Summary: Provides supportive services designed to prevent or reduce inappropriate institutional care by offering assistance to eligible recipients and the hiring, training and supervising responsibilities of the personal care aides, who perform basic health-related services to EDCD Waiver recipients.Essential Duties:* Ensure that clients are receiving the services needed and that they are on a current service plan.* Review required documents with the client, family and caregiver, as appropriate.* Train the caregivers providing care for consumer directed clients on the required tasks of an employer.* Develop service plans and complete appropriate Company/State required paperwork.
