Job Description:
Minimum Qualifications: Has lived experience receiving mental health services for serious mental illness and/or substance use conditions and has been in recovery for a minimum of 2 years. Uses life experience and recovery to provide knowledge and insight to clients to help them achieve and maintain recovery and wellness from mental health and/or substance use conditions. Education: High School Graduate or GED Certificate required. Must be able to read, write and comprehend oral instructions. Must be willing to use or learn computer software. Must have personal attributes suited to working in a team environment with persons with serious mental illness. Licensure: Capacity and willingness to achieve Florida Peer Recovery Specialist Certification. Experience: Must be able to assist clients with similar disorders, who have forensic involvement. Must be 21 years of age.Negative Drug Screening prior to hire and throughout employment.Clear Criminal Background Check throughout employment with Suncoast Center and local checks prior to hire and annually along with DCF Level II prior to hire and as required by DCF, and Must have valid Florida Driver's License Motor Vehicle checks prior to hire and annually with acceptable driving record maintained throughout employment along with reliable vehicle, valid registration, and valid vehicle insurance. (must be in compliance with Florida Laws and Administrative Codes with fingerprinting and determined to be of good moral character). Working Conditions: Must be able to assist clients with severe and persistent mental illness who fit chapter 916. Required to drive own car and/or company vehicle. Must meet established insurance requirements. Must have a driving record acceptable to Suncoast Center. FACT office, as well as in the community where most contacts occur. Exposure to extreme heat and humidity during the summer months. Computer Literate Job Duties:
  • Completes documentation correctly an accurately and in the required time frame including progress notes with objective to treatment plan clearly stated, time cards, and other documentation as requested by the team leader or needed to ensure all client records are complete.
  • Provides direct clinical services that use solution-oriented strategies of intervention and principles of recovery that are consistent with best practice principles, uses respectful and competency-based words and actions with clients and families while ensuring that client oriented treatment plan goals are incorporated that include symptom management by medication and promote personal growth and development.
  • Participates in daily staff organizational meetings and treatment planning review meetings (when time in schedule permits). Discusses cases appropriately at staffings. When not able to be present at a daily meeting, calls in with specific review of client contacts over the previous 24-hour period.
  • Provides specific FACT services depending on staff responsibilities designated by team leader including ensuring that client has safe and affordable place to live, necessary medical and dental services, financial support (employment and/or entitlements), ability to learn activities of daily living, vocational assistance, activities for leisure time, and other services client may request and have on individualized treatment plan.
  • As part of a multi-disciplinary team, cooperates and participates fully with the FACT Team Program to provide all necessary services to the clients. Demonstrates flexibility in support of the overall mission of the FACT Team Program.
  • Coordinates with client's family members, other FACT staff and community supports to ensure client needs are met and all involved with client's treatment work together.
  • Meets with team leader for supervision when needed; discuss cases appropriately and maintains excellence in all work performed.
