VISION: All New Mexico families and young children are thriving MISSION: Optimize the health, development, education, and well-being of babies, toddlers, and preschoolers through a family-driven, equitable, community-based system of high-quality prenatal and early childhood programs and services. The Early Childhood Education and Care Department was established on July 1, 2020. The Department's aim is to create a more cohesive, equitable, and effective early childhood system in New Mexico. That means coordinating a continuum of programs from prenatal to five and ensuring that families in every corner of the state can access the services they need. Why does the job exist? The purpose of the Registered Nurse III Families FIRST role is to provide comprehensive care coordination to Medicaid-eligible pregnant women and children up to age three. The care coordinator is directly involved in the assessment and evaluation of pregnant women and children ages 0-3 years. The Families FIRST RN III ensures the management, coordination, and monitoring of target population in areas of medical, social and educational needs to improve birth outcomes of pregnant women and the health status of their infants as outlined in the Families FIRST model. The Families FIRST RN III establishes a referral network and collaborates with community groups to educate clients on the importance of early prenatal care and preventive services for pregnant women and their infant children. The Families FIRST RN III provides education to clients on pregnancy and childhood development and refers clients to community resources to assist them in achieving full potential for their families. How does it get done? A Family Connects home visiting nurse provides home visiting to all families with newborns in the determined catchment area. The goal is to support and assess family needs in 12 domains of physical and psychosocial well-being and facilitate referrals and connections to community agencies to support families in these areas. The home visiting nurse will work in a team environment of clinical and non-clinical staff in order to schedule, visit, and support families in the community using the evidence-based Family Connects home visiting protocol. Ability to communicate effectively with others is essential. Experience in maternal/child health, home visiting, lactation education, and fluency in Spanish desired. Participation in program outreach at events, health fairs, conferences, and community baby showers are anticipated with this position. Who are the customers? As a community-based program, Family Connects supports new parents in caring for their newborn(s), offers physical assessments of the mother and the baby, addresses questions about caring for the newborn(s), identifies parent's needs, and helps to identify community services or resources that can meet those needs. Ideal Candidate The ideal candidate will have experience with pregnant women in an OB clinic or hospital setting such as Labor and Delivery. The ideal candidate will have experience with infants, toddlers and children, ages 0-3 yrs. Experience in case management and bilingual (Spanish) is preferred. The ideal candidate will be a self-starter and able to work independently as well as with a team. Minimum Qualification Must possess a current license to practice as a Registered Nurse in New Mexico OR eligibility for such licensure in accordance with New Mexico requirements OR Eligibility for a temporary license issued for employment at a specific institution in New Mexico and three (3) years of experience as it pertains to the essential duties and responsibilities of the classification. Employment Requirements Must possess and maintain a valid Driver's License. Pre-employment background investigation is required and is conditional pending results. Must possess a current license to practice as a Registered Nurse in New Mexico OR have a compact state license with eligibility for such licensure in accordance with New Mexico requirements and three (3) years of experience as it pertains to the essential duties and responsibilities of the classification. Must obtain and maintain valid CPR certification for healthcare providers. Must maintain registered nursing licensure per the NM Nurse Practice Act. Working Conditions Work is performed in an office setting with exposure to Visual/Video Display Terminal (VDT) and extensive personal computer and phone usage.
